盘钻法行上颌窦内提升牙种植术的长期临床效果评价 被引量:9

摘要 目的观察并评价盘钻法行上颌窦内提升牙种植术的长期临床效果。方法 2008年3月—2011年3月,我科应用盘钻法行上颌窦内提升术并同期植入种植体病人108例,植入种植体123颗。术后6~9个月行种植后修复,进行临床和X线检查并评估临床效果。结果 108例病人中有8例发生上颌窦黏骨膜穿破,同期行上颌窦外提升术,在观察期内未发生上颌窦炎等并发症;其余100例均末发生上颌窦黏骨膜穿破。术后X线检查显示,上颌窦底提升高度为2~8mm,平均4.6mm。随访中,123颗种植体中有2颗松动脱落,其余均获得良好的骨结合并行冠修复,咀嚼功能良好。结论盘钻法行上颌窦内提升牙种植术创伤小,操作简便,对于上颌后牙区垂直骨缺损的种植修复成功率高。 Objective To evaluate the long-term clinical effect of internal maxillary sinus floor elevation with simultaneous implantation by the disk-up sinus reamer(DSR). Methods A total of 108 patients underwent maxillary sinus floor elevation with simultaneous implantation by the DSR from March 2008 to March 2011.A total of 123 implants were placed.Post implant repair was carried out 6-9 months after surgery.The clinical effectiveness was evaluated through clinical examination and radiography. Results In the 108 patients of dental implant,maxillary sinus mucosa perforation was noted in eight patients,maxillary sinus lift operations were performed in the corresponding period,and no complications,such maxillary sinusitis and others,were found during the follow-up.Post-operative X-ray examination indicated that the elevation height of maxillary sinus floor ranged 2to8 mm,with an average of 4.6mm.During the follow-up,two of 123 implants were loosed and fell off,the rest of them gained osteointegration and restored with normal chewing function. Conclusion Internal maxillary sinus floor elevation with simultaneous implants placement by the DSR proves to be a less painful and easy to operate,with a high repair success rate.
