摘要
目的探讨二维应变超声成像技术对心力衰竭病人再同步化治疗(CRT)后左心室功能的评价价值。方法选取我院2013年1月—2014年1月收治的心力衰竭病人30例,均接受CRT,于术前、术后1个月、术后3个月测量左心室排血分数(LVEF)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD),并应用二维应变超声成像技术测量环向应变(CS)峰值、径向应变(RS)峰值及纵向收缩期应变(LS)峰值。结果 CRT后1、3个月LVEF、LVEDD、LVESD及LS、RS峰值与术前比较,差异有显著性(F=8.57~73.35,q=3.49~16.42,P〈0.05);CRT后1、3个月LVEF、LVEDD、LVESD和CS、LS、RS峰值比较差异无显著性(P〉0.05)。结论应用二维应变超声成像技术能够动态评价CRT后心脏结构与功能的变化,可用于评价CRT的短期疗效,具有较高的临床应用价值。
Objective To assess the value of two-dimensional strain ultrasonic imaging technology(2-DSUIT)for determining the left ventricular function after cardiac resynchronization therapy(CRT)for patients with heart failure. Methods From Jan.2013 to Jan.2014,30 patients with heart failure were treated in our hospital.All the patients received CRT.The left ventricular ejection fraction(LVEF),left ventricular end systolic dimension(LVESD)and left ventricular end diastolic dimension(LVEDD)were detected-before and after 1-month and 3-month CRT.The peak value of circumferential strain(CS),radial strain(RS)and longitudinal strain(LS)was measured using 2-DSUIT. Results A comparison between before and after CRT,the differences in The LVEF,LVEDD,LVESD and the peak of LS and RS were significant(F=8.57-73.35,q=3.49-16.42,P〈0.05),and the differences in that between 1-month and 3-month CRT were not significant(P〉0.05). Conclusion Dynamic evaluation of 2Dstrain ultrasonic imaging for the changes of heart structure and function,after treated with cardiac resynchronization therapy,has a high clinical application value in the assessment of the short-term effectiveness of the therapy.
