妊娠期糖尿病合并巨细胞病毒感染与凝血功能的相关性

Correlation analysis of gestational diabetes mellitus women with human cytomegalovirus infection and blood coagulation function

目的检测妊娠期糖尿病(gestational diabetes mellitus,GDM)患者中巨细胞病毒感染的发生率,探讨妊娠期糖尿病合并巨细胞病毒感染对妊娠妇女凝血功能的影响。方法妊娠期糖尿病患者158例作为试验组,136例健康孕妇作为对照组,检测人巨细胞病毒(human cytomegalovirus,HCMV)阳性感染率,根据是否感染,将以上两组又分为HCMV阳性组和阴性组。检测凝血酶原时间(prothrombin time,PT)、活化部分凝血活酶时间(activated partialthromboplastin time,APTT)以及纤维蛋白原(fibrinogen,Fib)变化,分析血糖升高和病毒感染对凝血功能的影响。结果妊娠期糖尿病患者HCMV阳性感染率29.11%,高于对照组7.35%,差异具有统计学意义(P<0.01)。单纯HCMV阳性感染健康孕妇组与阴性对照组比较,PT、APTT缩短,Fib升高,GDM合并HCMV感染与单纯血糖升高患者比较,PT、APTT缩短,差异有统计学意义(P<0.01)。结论妊娠期糖尿病妇女容易诱发HCMV感染,GDM孕妇合并HCMV感染可加重患者高凝血状态,存在血栓形成倾向。

Objective To investigate the infection rate of human cytomegalovirus（HCMV）in gestational diabetes mellitus（GDM）women,and the changes of coagulation function system in the patients with gestational diabetes mellitus and humancytomegalovirus infection. Methods A total of 158 GDM women were collected as an experiment group and 136 healthypregnant women as a control group, HCMV infection were examined,according to whether the infection, the above groups weredivided into HCMV positive and negative group. Prothrombin time（PT）,activated partial thromboplastin time（APTT）,andfibrinogen（Fib）were examined,and the effects of blood glucose and virus infection on coagulation function were evaluated.Results In the GDM patients,the HCMV infection rate was 29.11%,higher than 7.35% of the control group（P0.01）. PTand APTT were significantly shorter,and Fib was significantly higher in the simple HCMV infection pregnant women sub-group compared with those in the control group. PT and APTT were significantly shorter in the GDM and HCMV infection sub-group compared with those in the simple GDM sub-group（P〈0.01）. Conclusion The GDM women are prone to inducingHCMV infection. The high blood coagulation state is aggravated in the GDM women with HCMV infection,and there is atendency of thrombosis.
