超声引导经皮穿刺置管引流术治疗肝脓肿患者的效果

Ultrasound-guided percutaneous catheter drainage in treatment of liver abscess

目的研究超声引导经皮穿刺置管引流治疗肝脓肿患者的效果。方法选取2010年2月至2015年2月本院收治的98例肝脓肿患者，以数字法随机分成观察组（49例）和对照组（49例）。观察组超声引导经皮穿刺置管引流术治疗，对照组切开置管引流术治疗。观察两组治疗效果以及并发症等情况。结果观察组显效率为87．76％（43／49），优于对照组的71．43％（35／49）（χ2=4．021，P=0．045）；观察组总有效为100．00％（49／49）优于对照组的91．84％（45／49）（χ2=4．170，P=0．041）；观察组白细胞数、体温恢复正常时间，拔管时间及住院时间均显著低于对照组（t=7．259，P=0．000；t=5．585，P=0．000；t=6．212，P=0．000；t=7．314，P=0．000）；观察组并发症发病率为2．04％，显著低于对照组的14．29％（χ2=4．900，P=0．027）。结论超声引导经皮穿刺置管引流术治疗肝脓肿患者效果好，能加快患者术后恢复，显著降低术后并发症情况，值得推广。

Objective To study the effect of ultrasound-guided percutaneous catheter drainage for liver abscess. Methods 98 patients with liver abscess treated at our hospital from February, 2010 to February, 2015 were selected and randomly divided into an observation group and a control group, 49 for each group. The observation group took ultrasound-guided percutaneous catheter drainage and the control group incision and catheter drainage. The treatment effect and complications were observed in both groups. Results The effective rate and total efficacy were better in the observation group than in the control group [87.76% （43/49） vs. 71.43% （35/49）, χ2=4.021, P=0.045 and 100.00% （49/49） vs. 91.84% （45/49）,χ2=4.170,P=0.041]. The white blood cell count was lower and the time for body temperature returning to normal, the time for extubation, and hospital stay were significantly shorter in the observation group than in the control group （ t=7.259, P=-0.000; t=5.585, P=0.000; t=6.212, P=0.000; t=7.314, P=0.000 ） . The incidence of complications was 2.04% in the observation group and 14.29% in the control group （χ2= 4.900, P=0.027）. Conclusions Ultrasound-guided percutaneous catheter drainage for patients with liver abscess is effective and can promote the patients＇ post-operative recovery and decrease the incidence of post-operative complications, so it is worth being generalized.
