微创漏斗胸矫形术治疗漏斗胸的效果观察

Effect observation of minimally invasive funnel chest orthopedics in the treatment of funnel chest

目的探讨微创漏斗胸矫形术治疗漏斗胸的效果。方法选取我院2010年1月~2015年12月收治的75例先天性漏斗胸患者作为研究对象,按照手术方式的不同分为对照组（45例）和观察组（30例）。对照组采用传统手术方式治疗,观察组采用微创漏斗胸矫形术治疗。比较两组的手术情况（手术时间、术中出血量、住院天数）及患者手术前后的心肺功能[第1秒用力呼气容积（FEV1）、1 min内的最大通气量（MVV）、心指数和左心室射血分数、每搏输出量和心排血量]改善情况。结果观察组的手术时间、术后住院天数短于对照组,术中出血量少于对照组,术后疼痛程度低于对照组,差异有统计学意义（P〈0.05）。两组术后的心肺功能指标均优于术前,差异有统计学意义（P〈0.05）。两组术后的心肺功能指标比较,差异无统计学意义（P〉0.05）。结论微创漏斗胸矫形术治疗漏斗胸的矫正效果好,创伤小,出血少,手术时间短,患者术后恢复快,值得临床推广应用。

Objective To explore the effect of minimally invasive funnel chest orthopedics in the treatment of funnel chest.Methods 75 cases of congenital funnel chest in our hospital from January 2010 to December 2015 were selected and divided into the control group（45 cases） and the observation group（30 cases） according to the different operation method.The control group was treated with traditional surgical treatment,the observation group was treated with minimally invasive funnel chest orthopedics.The operation conditions（operation time,intraoperative blood loss,hospitalization days）,the improvement situation of cardiac and pulmonary function（FEV1,MVV,cardiac index and left ventricular ejection fraction,stroke volume and cardiac output） before and after surgery in the two groups was compared.Results The operation time and postoperative hospital stay in the observation group was shorter than that in the control group,the amount of bleeding in the observation group was less than that in the control group,the degree of postoperative pain in the observation group was lower than that in the control group,with significant difference（P〈0.05）.The cardiac and pulmonary function indexes of the two groups after operation was better than that before operation,with significant difference（P〈0.05）.There was no significant difference in the cardiac and pulmonary function indexes after operation between the two groups（P〉0.05）.Conclusion The correction effect of minimally invasive funnel chest orthopedics is good,with less trauma,less bleeding,shorter operation time,and postoperative recovery of patients is fast,it is worthy of clinical promotion and application.
