摘要
目的探讨血液透析滤过（HDF）、血液透析（HD）联合血液灌流（HP）对尿毒症周围神经病变患者感觉定量检测（QST）阈值的影响,评价HDF、HP治疗尿毒症周围神经病变的效果。方法选取我院2013年1月~2015年10月收治的52例尿毒症周围神经病变HD患者作为研究对象,随机分为HDF组和HD＋HP组,各26例。HDF组给予每周2次HD和1次HDF治疗,HD＋HP组给予每周2次HD和1次HD＋HP治疗。分别于治疗前、治疗4个月后观察两组的周围神经病变临床症状,比较两组左侧手指、小鱼际、小腿及足背4个不同部位QST的温度觉阈值及振动觉阈值。结果两组治疗4个月后的周围神经病变临床症状及4个不同部位的温度觉阈值、振动觉阈值显著优于治疗前,差异有统计学意义（P〈0.05）。HD＋HP组治疗后的温度觉阈值改善程度显著优于HDF组,差异有统计学意义（P〈0.05）。结论 HD＋HP、HDF均能有效改善尿毒症周围神经病变。
Objective To explore the influence of hemodiafiltration（HDF）,hemodialysis（HD） combined with hemoperfusion（HP） on the threshold of quantitative sensory testing（QST） in uremic patients with peripheral neuropathy,to evaluate the effect of HDF,HD＋HP in the treatment of uremic peripheral neuropathy.Methods 52 uremic patients with peripheral neuropathy in our hospital from January 2013 to October 2015 were selected and randomly divided into the HDF group and the HD＋HP group,26 cases in each group.The HDF group was given regular HD 2 times a week and HDF once a week,the HD ＋HP group was given regular HD 2 times a week and HD ＋HP once a week.The clinical symptoms of peripheral neuropathy in the two groups before and after treatment for 4 months was observed respectively,the QST temperature threshold and vibration perception threshold of 4 different parts such as the left finger,hypothenar,leg and foot in the two groups was compared.Results The clinical symptoms of peripheral neuropathy,the QST temperature threshold and vibration perception threshold of 4 different parts after treatment for 4 months in the two groups was better than that before treatment,with significant difference（P〈0.05）.The improvement degree of temperature threshold after treatment in the HD＋HP group was better than that in the HDF group,with significant difference（P〈0.05）.Conclusion HD＋HP,HDF can effectively improve peripheral neuropathy of uremic patients.
