腹腔镜开窗引流治疗肝囊肿的临床应用

Clinical Application of Laparoscopic Fenestration and Drainage in the Treatment of Hepatic Cyst

目的：观察腹腔镜开窗引流治疗肝囊肿的临床效果。方法：研究对象源自某院2013年11月-2014年11月收治的66例肝囊肿患者,随机分为对照组（33例）与观察组（33例）。对照组患者采用开腹手术治疗;观察组患者采用腹腔镜开窗引流术治疗。统计两组患者手术时间等手术指标,观察术后并发症发生情况,随访6个月,记录复发病例。结果：两组患者均顺利通过手术,观察组患者手术时间（75.26±15.13）min、术中出血量（55.38±11.62）ml、术后引流量（63.62±11.25）ml、肠道功能恢复时间（17.86±2.53）d及住院天数（9.62±3.27）d均优于对照组（P〈0.05）;观察组患者出院后无复发病例,对照组复发率为12.1%,两组比较差异有统计学意义（P〈0.05）;两组患者术后并发症发生率9.0%和6.0%。组间对比无显著差异（P〉0.05）。结论：腹腔镜开窗引流治疗肝囊肿创伤小,出血少,术后无严重并发症,能有效降低复发率,疗效安全可靠。

Objective：To observe the clinical effect of laparoscopic fenestration and drainage in the treatment of hepatic cyst.Methods：66patients with hepatic cyst treated in our hospital from November 2013 to November 2014 were taken as the research objects and were randomly divided into the control group（33cases）and the observation group（33cases）.The control group was treated with open operation,while the observation group was treated with laparoscopic fenestration and drainage.The operation indexes such as operation time of the two groups were statistically analyzed and the occurrence of postoperative complications was observed.Followed up for 6months,the recurrent cases were recorded.Results：The two groups of patients were successfully operated.The operation time（75.26±15.13）min,intraoperatve bleeding volume（55.38±11.62）ml,postoperative drainage volume（63.62±11.25）ml,time for intestinal function recovery（17.86±2.53）d and length of hospital stays（9.62±3.27）d of the observation group were better than those of the control group（P〈0.05）;There was no recurrent cases in the observation group after discharge and the recurrence rate in the control group was 12.1%（P〈0.05）;The incidence rates of postoperative complications in the two groups were 9.0% and 6.0%（P〉0.05）.Conclusion：Laparoscopic fenestration and drainage in the treatment of hepatic cyst is with small trauma,less bleeding and without serious postoperative complications,which can effectively reduce the recurrence rate.The curative effect is safe and reliable.
