超声在危重症患者肠梗阻病因分型诊断中的应用价值观察

Application Value of Ultrasound in Etiology Classification Diagnosis of Critical Patients with Intestinal Obstruction

目的：对危重症患者肠梗阻原因进行分析以及对分型诊断时应用超声的效果进行探究。方法：选择某院于2013年6月-2015年6月收治的50例危重症同时发生肠梗阻的患者,对其均予以超声检查,对患者的病因进行分型诊断,并对诊断的假阳性率进行分析。结果：50例患者中共有29例患者经超声检查后发现病因,超声检查的符合率为56.00%,其中发生在小肠位置,病因为肠套叠与腹股沟疝的肠梗阻的超声检查结果符合率较高,能够达到100.00%,而因肠系膜血管壁闭塞导致的肠梗阻的检查符合率却相对较低,为20.00%,二者比较χ^2=6.67,P=0.01〈0.05,差异在统计学中存在意义。结论：超声检查能够对危重症肠梗阻进行病因分型诊断,但是其检查结果可能会受到气体的干扰,同时超声检查很难对回肠与空肠的界限进行判断,因此诊断结果会有一定的误差。但超声图像的显示较为直观,且在诊断后可辅助医师及时选定治疗方案,因此值得对其进行深入研究并应用。

Objective：To explore the causes of critically ill patients with intestinal obstruction and to analyze the effect of ultrasound in the diagnosis of the disease.Methods：50cases of critical patients with intestinal obstruction treated in our hospital from June 2013 to June 2015 were examined by ultrasonography.The cause of the disease was diagnosed and the false positive rate was analyzed.Results：29cases of all patients were found the cause after the ultrasonic inspection,and the ultrasound coincidence rate was 56.00%.The ultrasound coincidence rate of the cause which was intussusception with inguinal hernia of intestinal obstruction and occurred in the position of the small intestine was 100.00%.Compared with the former,the ultrasound coincidence rate of the cause which was mesenteric blood vessels occlusion was significantly lower,which was just 20.00%.The difference between the two causes was statistically significant（χ^2=6.67,P=0.01 〈0.05）.Conclusion：Ultrasonic examination can diagnose the cause of critically ill patients with intestinal obstruction,but the results may be affected by gas interference,and it is difficult to judge the limits of ileum and jejunum.However,the ultrasound image display is more intuitive,and can assist doctors to select the treatment plan in time,so it is worth to be further studied and applied.
