伤椎置钉前柱打压植骨治疗胸腰椎爆裂骨折疗效观察

Compress Bone Graft for Front Post to Treat Thoracolumbar Burst Fractures

目的探讨胸腰椎爆裂性骨折经伤椎椎弓根打压植骨的可行性。方法自2009-03—2015-08间对深圳市光明新区人民医院骨科的176例胸腰椎爆裂性骨折患者,给予伤椎及上下邻椎椎弓根固定治疗,并进行后路减压,经伤椎椎弓根向椎体前柱植骨并打压填实。结果术后椎体高度基本恢复正常,术后后凸角度基本纠正,经过6~24个月随访,未发现椎体高度丢失及内固定物折断。结论经椎弓根向伤椎前柱打压植骨,使前柱获得良好支撑,防止术后椎体高度丢失,减少内固定的松动或断裂。

Objective To investigate the feasibility of transpedicular impaction bone grafting through fractured vertebra in the treatment of thoracolumbar burst fracture. Methods From March,2009 to August,2015,176 patients of thoracolumbar burst fracture were treated with internal pedicle fixation,posterior decompression and impacted bone grafting to anterior column through the pedicle of fractured vertebra. Results After the operation,patients＇ vertebral height basically recovered to normal and the kyphosis was corrected mostly. No vertebral height loss or implants failure was found after 6 ~ 24 months following up. Conclusion Transpedicular impaction bone grafting to anterior column of fractured vertebra can provide sufficient support consequently,prevent centrum from height losses,and reduce the possibility of loosening and breakage of internal fixation.
