脾胃虚寒型晚期胃癌行附子理中汤加减联合化疗对外周血T细胞及生活质量影响

Effect of aconite lizhong decoction combined with chemotherapy on peripheral blood T cells and life quality of advanced gastric cancer due to deficiency cold of spleen and stomach

[目的]分析附子理中汤加减联合化疗对晚期胃癌脾胃虚寒型患者外周血T细胞及生活质量影响。[方法]选取我科2013年6月~2014年6月内镜下或术中病理诊断明确,的晚期胃癌患者共120例。随机数字表法将患者分为实验组和对照组,对照组予以奥沙利铂+卡培他滨化疗,实验组在对照组基础上,加用附子理中汤加减治疗。对比两组患者治疗前和治疗两个周期后外周血T细胞、卡氏评分(Karnofsky评分,KPS)和生活质量评定(Quality of life,QOL)。对比2组患者治疗2个周期后的近期疗效和不良反应发生率。[结果]2组患者治疗2个周期后的CD^(3+)、CD^(4+)和CD^(4+)/CD^(8+)细胞水平、KPS和QOL评分显著高于治疗前,P<0.05;实验组疗2个周期后的CD^(3+)、CD^(4+)和CD^(4+)/CD^(8+)细胞水平、KPS和QOL评分显著高于对照组,P<0.05。实验组和对照组治疗后近期疗效差异无统计学意义,P>0.05。2组患者化疗不良反应发生率差异无统计学意义,P>0.05。[结论]晚期胃癌患者化疗时加用附子理中汤加减可显著改善患者免疫水平和生活质量。

[Objective]To analyze the effect of aconite decoction combined with chemotherapy for Advanced gastric cancer with stomach Deficiency type in peripheral blood T cells and quality of life.[Methods]120cases of patients with advanced gastric cancer in our department from June 2013 to June 2014 with endoscopic or intraoperative pathological diagnosis were randomly divided into experimental and control groups.The control group was treated with oxaliplatin ＋ capecitabine chemotherapy.The experimental group had the addition of Aconite Decoction.The peripheral blood T cells,KPS（Karnofsky score,KPS）and Quality of Life Assessment（Quality of life,QOL）were compared in two groups of patients before treatment and after two cycles of treatment.The efficacy and adverse reaction rate after two cycles of treatment were compared in two groups of patients.[Results]CD^（3＋）、CD^（4＋）and CD^（4＋）/CD^（8＋）cell levels,KPS and QOL scores in the two groups of patients 2cycles later were significantly higher than before treatment,P0.05.CD^（3＋）、CD^（4＋）and CD^（4＋）/CD^（8＋）cell levels,KPS and QOL scores in the experimental treatment 2cycles later was significantly higher,P0.05.There was no significant difference in short term curative effect between the experimental group and the control group,P0.05.Two groups of patients with chemotherapy incidence of adverse reactions had no significant difference,P0.05.[Conclusion]Chemotherapy plus Lizhong decoction can significantly improve the quality of life of patients and immunity levels in patients withadvanced gastric cancer.
