鸦胆子油乳联合复方苦参辅助治疗对结直肠癌根治术患者预后的影响

Influence of bruceolic oil emulsion combined with compound matrine treatment on prognosis of patients with radical resection of rectal carcinoma

目的探讨鸦胆子油乳注射液联合复方苦参注射液对结肠癌根治术患者预后的影响。方法选取2010年1月至2012年1月医院结直肠癌根治术患者83例作为研究对象，术后均给予常规化疗，随机分为两组，观察组40例给予鸦胆子油乳注射液联合复方苦参注射液治疗，对照组43例给予复方苦参注射液治疗，行功能状态评分（KPS）评分、欧洲癌症研究与治疗组织E-ORTC制定的肿瘤患者生存质量量表EORTC QLQ LC43评分；门诊或电话随访3年，记录两组术后1年、2年、3年疾病复发转移率；对比两组T淋巴细胞水平和化疗毒副反应。结果观察组改善率为80．00％，高于对照组（58．14％）（P〈0．05）；观察组2年复发转移率为5．00％，低于对照组（20．93％）（P〈0．05）；观察组Ⅰ-Ⅳ级白细胞下降、恶心呕吐发生率低于对照组，差异具有统计学意义（P〈0．05）；观察组治疗后EORTC QLQ LC43评分亚型躯体功能、情绪功能、认知功能、角色功能、社会功能、健康经济相关、症状因子、结直肠癌特异性模块评分低于对照组（P〈0．05）；观察组治疗后CD4＋、CD4＋／CDS＋高于对照组，CD8＋低于对照组（P〈0．05）。结论鸦胆子油乳注射液联合复方苦参注射液能够减轻结直肠癌根治术后化疗毒副反应。改善免疫功能。且有利于改善个体功能状态和生活质量。

Objective To investigate the influence of bruceolic oil emulsion combined with compound matrine on prognosis of patients with radical resection of rectal carcinoma. Methods 83 cases given radical resection of rectal carcinoma in our hospital from January 2010 to January 2012 were selected as study objects. They were divided into two groups randomly, 40 cases in observation group and they were given bruceolic oil emulsion combined with compound matrine, 43 cases in control group and they were given compound matrine infection. They were given KPS score and EORTC QLQ LC 43 score; outpatient or telephone follow-up for 3 years, recorded the recurrence and metastasis rate of the two groups after 1,2 and 3 years; compared the T lymphocyte level and chemotherapeutic toxic side effect of the two groups. Results Improvement rate of observation group （80.00%） was obviously higher than control group （58.14%）, the difference was statistically significant （P〈0.05）; recurrence and metastasis rate after 2 years of observation group （5.0%） was obviously lower than control group （20.93%）, the difference was statistically significant （P〈0.05）; Ⅰ-Ⅳ level leukocyte declined of observation group, and nausea and vomiting occurrence rate obviously lower than control group, the difference was statistically significant （P〈0.05）; EORTC QLQ LC 43 score, subtype body function,emotional function, cognitive function, role function, social function, health financial related, symptom factors, colorectal cancer specificity score of observation group were obviously lower than control group, the difference was statistically significant （P〈0.05）; CD4＋, CD4＋/CD8＋ in the observation group after the treatment were significantly higher than the control group, CD8＋ was lower than the control group （P〈0.05）. Conclusion Bruceolic oil emulsion combined with compound matrine injection can reduce chemotherapeutic toxic side effect, improve immunologic function, is conducive to improving functional status and quality of life.
