三维斑点追踪技术评价妊娠高血压综合征患者左心室收缩功能 被引量:2

摘要 目的运用三维斑点追踪成像技术(3D-STI)评价妊娠高血压综合征(PIH)患者左心室整体收缩功能,探讨其早期改变及临床应用价值。方法 32例PIH患者和40例健康孕妇作为观察和对照组,使用3D-STI测量观察及对照组左心室整体纵向收缩期峰值应变(LVGLS)、左心室整体径向收缩期峰值应变(LVGRS)、左心室整体环向收缩期峰值应变(LVGCS)、左心室整体面积收缩期峰值应变(LVGAS)及左心室射血分数(LVEF),并做比较。结果与正常对照组相比较,观察组LVGLS减低(P<0.05),LVGRS、LVGCS、LVGAS、LVEF无明显变化(P>0.05)。结论 3D-STI技术通过评估PIH患者左心室的三维整体应变,可敏感反映左心室整体收缩功能的改变。PIH患者在LVEF正常时已发生收缩功能改变,为临床早期治疗及疗效的评价提供参考依据。 Objective To evaluate the early changes and clinical value of left ventricular systolic function in patients with pregnancy-induced hypertension(PIH) by three-dimensional speckle tracking imaging(3D-STI). Methods 32 PIH patients and 40 healthy control subjects were included. left ventricular global peak longitudinal strain(LVGLS),left ventricular global peak radial strain(LVGRS),left ventricular global peak circumferential strain(LVGCS),left ventricular global peak area strain(LVGAS) and left ventricular ejection fraction(LVEF) were measured by 3D-STI,compare the differences between the two groups. Results Compared with the control group,LVGLS decreased in observational group(P〈0.05),no significant differences in LVGRS,LVGCS,LVGAS and LVEF among the two groups. Conclusions 3D-STI technique can sensitively reflect the change of left ventricular systolic function in patients with PIH by assessing global response of left ventricle. PIH patients may have systolic function change with normal LVEF. 3D-STI technique provides a reference for clinical treatment and efficacy evaluation.
