摘要
目的探讨Ki-67在年龄≥60岁患者三阴性乳腺癌(TNBC)组织中的表达情况,以及与临床病理特征和相关因子的关系。方法收集病理资料完整的非特殊型浸润性癌的TNBC病例99例,其中年龄〉160岁(老年组)55例,年龄〈60岁(非老年组)44例。采用免疫组织化学SP法检测Ki-67、GST-π和Top-Ⅱ的蛋白表达情况,分析Ki-67表达与年龄、原发肿瘤大小、腋窝淋巴结转移及病理分级等的相关性,以及与GST-π和Top-Ⅱ的关系。结果TNBC患者中81例Ki-67阳性(阳性细胞指数≥10%),18例Ki-67阴性(阳性细胞指数〈10%)。老年组TNBC的Ki-67阳性率为83.33%,明显高于非老年组的70.45%(P〈0.05)。Ki-67阳性表达与病理组织学分级呈明显正相关性,差异有统计学意义(P〈0.05)。非老年组TNBC中,在临床分期Ⅰ期的Kj-67表达阳性率明显高于临床分期Ⅱ-Ⅳ期者,差异有统计学意义(P〈0.05)。老年组TNBC中Ki-67阳性表达率与GST一百的表达差异有统计学意义(P〈0.05),而在非老年组中则差异无统计学意义(P〉0.05)。结论老年组TNBC组织中Ki-67高表达提示癌组织分化更差,且与GST-π相关化疗药耐药有关,检测TNBC中Ki-67表达对判断乳腺癌的恶性程度及指导治疗有一定的参考意义。
Objective To detect the expression of Ki67 in triple negative breast cancer (TNBC) of elder woman (age≥60 years), and analyze the relationship with clinicopathological charactersin order to supply experimental basis of standard therapy for elder TNBC patients. Methods 99 cases of TNBC clinicopathologica datas were obtained from General Hospital of Ningxia Medical University. The immunohistochemistry was explored to detect the protein expression of Ki - 67, GST - π and Top - Ⅱ in TNBC. The relationship between Ki- 67 expression and the size of tumor, the metastasis of axillary lymph nodes, clinical stages, pathological grades, as well GST - π and Top - Ⅱ expressions of TNBC were analyzed. Results 81 cases of TNBC had Ki - 67 positive expression, while 18 cases had Ki -67 negative expression. The expression of Ki -67 was higher in elder group (age≥60 years) than non -elder group (age 〈 60 years). There was significantly positive association and difference between Ki - 67 expression and pathological grades ( P 〈 0.05 ). In non - elder TNBC group, the expression of Ki - 67 was obviously higher in clinical stage I than that in stage Ⅱ - Ⅳ ( P 〈 0.05 ). There was significant difference between Ki - 67 expression and GST - π 、Top - Ⅱ expressions in elder TNBC ( P 〈 0.05 ). Conclusions The expression of Ki - 67 is obviously increased in elder TNBC group than that in non - elder group. The higher expression of Ki - 67 may be associated with poorer differentiated in TNBC. Theincreased expression of Ki67 may possibly resistthe chemical therapy in elder TNBC. There was promising value on the judge of malignant degree and prognosis, as well guide of therapy to detect the expression of Ki - 67 in TNBC.
