摘要
目的探讨继发性红细胞增多症(SP)对慢性阻塞性肺疾病(COPD)男性患者呼吸功能和骨髓有核红细胞凋亡及Bcl-2 mRNA的影响。方法分别选取30名单纯骨折男性患者和32名COPD男性患者、21名COPD+SP男性患者作为对照组和COPD组、COPD+SP组。用肺功能仪等仪器评价患者呼吸功能。用流式细胞仪测定骨髓有核红细胞凋亡率、RT-PCR法检测Bcl-2 mRNA的表达水平。结果 COPD+SP组肺功能(FEV1/FVC%)低于COPD组,而m MRC呼吸困难评价分级评分大于COPD组(P<0.05);COPD+SP组骨髓有核红细胞凋亡率明显低于对照组和COPD组(P<0.05),而Bcl-2 mRNA表达明显高于对照组和COPD组(P<0.05)。结论 SP影响COPD男性患者的肺功能,加重呼吸困难的程度。Bcl-2家族分子可能参与了COPD+SP男性患者骨髓有核红细胞凋亡异常的机制。
Objective The aim of the study was to investigate the impact of secondary polycythemia( SP) on respiratory function,bone marrow nucleated red blood cells apoptosis and Bcl-2 mRNA expression in male patients with chronic obstructive pulmonary disease( COPD). Methods To select 30 male simple fracture patients as control group,32 male COPD patients and 21 male COPD complicated with SP patients as COPD group and COPD + SP group. Pulmonary function( FEV1 / FVC) of patients were evaluated using the spirometer. The apoptosis rate of bone marrow nucleated red blood cells were measured by flow cytometry. Expression levels of Bcl-2 mRNA were detected by RT-PCR. Results Respiratory function( FEV1 / FVC) of COPD +SP group were lower than COPD group and the m MRC score were greater than that of COPD group( P〈0. 05). Apoptosis rate of bone marrow nucleated red blood cells with COPD+SP were significantly lower than that of the control group and COPD group. The expression levels of Bcl-2 mRNA were significantly higher than that of the control group and COPD group( P〈0. 05). Conclusions SP can affect the male respiratory function( FEV1 / FVC) and increase the degree of dyspnea in COPD patients. Bcl-2 family molecules may be involved in abnormal apoptosis of marrow nucleated red blood cells in male patients with COPD complicated with SP.
