苦参素对三维适形放射治疗中晚期肺癌疗效及血清VEGF、MMP-9水平的影响

Influence of Kurarinone on the patients with medium and advanced lung cancer treated with three-dimensional conformal radiation therapy and their serum VEGF and MMP-9 levels

目的探讨苦参素(0MT)对三维适形放射治疗中晚期肺癌临床疗效及患者血清VEGF、MMP-9水平的影响。方法将136例中晚期肺癌患者临床资料,按照治疗方案分为对照组(62例)和观察组(74例)。对照组患者给予三维适形放射治疗,观察组在对照组基础上给予0MT治疗。比较两组中晚期肺癌患者近期疗效、毒副反应及血清VEGF、MMP-9水平。结果两组中晚期肺癌患者近期疗效评价结果显示,观察组总有效率高于对照组(75.7%vs 56.4%,P<0.05)。治疗期间,观察组患者骨髓抑制、胃肠道反应、肝功能损害毒副反应发生率均低于对照组(P<0.05)。治疗后,与对照组相比,观察组患者血清VEGF、MMP-9水平降低(347.3±38.2 vs 404.5±42.0ng/L,476.8±47.3 vs 512.2±53.5ng/L),组间比较均有统计学差异(P<0.05)。结论在三维适形放疗基础上给予0MT治疗可以提高中晚期肺癌近期疗效,降低毒副反应发生率,而且能够降低血清VEGF、MMP-9水平,临床上值得进一步研究。

Objective To investigate the influence of Kurarinone on the patients with medium and advanced lung cancer treated with three- dimensional conformal radiation and their serum vascular epithelial growth factor（ VEGF） and matrix metalloproteinase-9（ MMP-9） levels. Methods Divided 136 medium and advanced lung cancer patients into control group（ n = 62） and the trial group（ n = 74） according to therapeutic programs. The control group was given three-dimensional conformal radiation therapy only,whereas the trial group was given three-dimensional conformal radiation therapy and Kurarinone treatment. Then,both groups＇ short-term curative efficacy,toxic side effects and levels of serum VEGF,MMP- 9 were compared. Results After the course of treatment,the short- term curative efficacy ratio in the trial group was 75. 7%,which was statistically higher than that of 56. 4% in the control group（ P〈0. 05）. As to toxic side effects,the incidences of myelosuppression,gastrointestinal reaction,liver function damage in the trial group ware statistically lower than those in the control group（ P〈005）. After the treatment,in comparison with the control group,the trial group showed statistically lower levels of serum VEGF and MMP- 9（ 347. 3±38. 2 vs 404. 5±42. 0ng /L,476. 8±47. 3 vs 512. 2±53. 5ng /L,respectively,both P〈0. 05）. Conclusion For the patients with medium and advanced lung cancer received three-dimensional conformal radiation therapy and addition to Kurarinone treatment can significantly increase the short-term efficacy and decrease the incidences of toxic side effects and reduce the levels of serum VEGF and MMP-9.
