无创呼吸机辅助肌肉训练对稳定期COPD患者生存质量影响的研究

Research in Influence of Noninvasive Ventilator Assisted Muscle Training on Quality of Life of Chronic Obstructive Pulmonary Disease in Stable Stage

目的:探讨无创呼吸机辅助肌肉训练的干预治疗对慢性阻塞性肺疾病(COPD)稳定期患者的疗效观察。方法:选择2014年8月-2016年4月在门诊及住院治疗的稳定期CODP患者56例,随机分为对照组和治疗组。对照组30例稳定期COPD患者进行常规内科治疗,治疗组26例患者除内科治疗外,使用无创呼吸机辅助通气联合呼吸肌锻炼、上下肢肌力训练等进行干预治疗,6个月后评价两组的动脉血气分析、肺通气功能(FEV_1、FEV_1%)、6 min步行试验、生存质量评估(通过圣乔治呼吸问卷及呼吸困难评分表)、研究期间急性加重次数、平均住院次数及平均住院时间。结果:治疗6个月后,治疗组PaO_2、PaCO_2、FEV_1、6 min步行距离、MMRC及SGRQ评分等,以及平均住院次数、平均住院时间与对照组比较,差异均有统计学意义(P<0.05)。结论:无创呼吸机辅助肌肉训练的康复治疗方法可降低COPD患者的PaCO_2及住院率,改善患者的肺功能,提高患者生存质量。

Objective： To evaluated the effects of intervention with noninvasive ventilator assisted muscle training on chronic obstructive pulmonary disease （ COPD ） in stable stage.Method ： A randomized controlled trial was performed from August 2014 to April 2016 in 56 patients with （ COPD ） in stable stage.They were randomly divided into two groups, the control group of 30 cases were given regular medical treatment, the treatment group of 26 cases were given regular medical treatment and noninvasive ventilator assisted muscle training.The 2 groups were compared in terms of arterial blood gas tests, pulmonary function （ FEV1, FEV1% ）, 6-minutes walk test, quality of life assessment scale （ St George＇s respiratory questionnaire and MRC ）, acute exacerbation average times, hospitalization average times and hospital stay time.Result： At 6 months after surgery, there were differences in PaO2, PaCO2, FEV1, 6-minutes walk test, SGRQ, MRC, hospitalization average times and hospital stay between the two groups （P〈0.05） .Conclusion： Noninvasive ventilator assisted muscle training has advantages of reducing PaCOa, hospitalization rate in COPD patients, improving the patients＇ lung function and the quality of life.
