摘要
目的探讨全身PET/CT显像对非小细胞肺癌(NSCLC)纵隔淋巴结转移的诊断价值。方法对85例NSCLC患者的临床资料作回顾性分析,术前均行PET/CT检查,并行手术根治或纵隔淋巴结活检。结果病理结果示23例发生淋巴转移,阳性率为27.1%。PET/CT对纵隔淋巴结转移的灵敏度为82.6%,特异度为90.3%,阳性预测值为76%,阴性预测值为93.3%,准确性为88.2%。结论PET/CT对纵隔淋巴结转移的特异度及阴性预测值较高,但灵敏度及阳性预测值欠佳,提示PET/CT检出为阳性的淋巴结需进一步行纵隔镜检查。
Objective To investigate the value of whole body PET/CT imaging in the diagnosis of mediastinal lymph node metastasis of non small cell lung cancer(NSCLC). Methods The clinical data of 85 patients with NSCLC were retrospectively analyzed. All patients underwent PET/CT examination before operation, and underwent radical or mediastinal lymph node biopsy. Results Pathological results showed that 23 cases had lymph node metastasis, the positive rate was 27.1%. The sensitivity of PET/CT to mediastinal lymph node metastasis was 82.6%, the specificity was 90.3%, the positive predictive value was 76%, the negative predictive value was 93.3%, the accuracy was 88.2%. Conclusion The specificity and negative predictive value of PET/CT for mediastinal lymph node metastasis were higher, but the sensitivity and positive predictive value were not well. It was suggested that PET/CT was positive for lymph nodes to be further performed in the diagnosis of mediastinal lymph node.
