5115例妇女生殖道B群链球菌的检测及药敏分析

Prevalence and antimicrobial susceptibility of group B Streptococci in reproductive tracts of women

目的研究妇女生殖道B群链球菌(GBS)的检出率及药物敏感性分析。方法对温州医科大学附属第一医院5115例妇女生殖道标本进行病原菌培养,重点对检到的B群链球菌进行药物敏感性试验及对该标本相应阴道分泌物进行分析。结果 5115例妇女生殖道标本中共培养B群链球菌180株,检出率为3.52%(180/5115例)。180株B群链球菌对万古霉素、替考拉宁、奎奴/达福普汀、替加环素、氨苄西林、青霉素的敏感率均为100.00%(180/180例),对呋喃妥因的敏感率为92.22%(166/180例),对莫西沙星的敏感率为82.22%(148/180例),对左氧氟沙星的敏感率为81.11%(146/180例),对环丙沙星的敏感率为55.00%(99/180例),对红霉素的敏感率为22.22%(40/180例),对克林霉素的敏感率为10.00%(18/180例),对四环素的敏感率为7.78%(14/180例)。携带B群链球菌的妇女阴道分泌物的常规检测的结果显示,阴道分泌物p H值以p H>4.40为主,为77.78%(140/180例);清洁度以Ⅲ度为主,为54.44%(98/180例)、Ⅳ度为17.78%(32/180例);滴虫为5.56%(10/180例),真菌为12.22%(22/180例),线索细胞为8.89%(16/180例),唾液酸苷酶阳性率为13.33%(24/180例),过氧化氢阳性率为98.89%(178/180例),白细胞酯酶阳性率为95.56%(172/180例)。结论妇女生殖道B群链球菌携带率相对较低,B群链球菌对常用抗菌药物敏感性较高,携带B群链球菌妇女基本上具有阴道炎症反应。

Objective To investigate the prevalence and antimcirobial susceptibility of group B Streptococci（GBS） in the reproductive tracts of women.Methods A total of 5115 specimens from the reproductive tracts of women from the First Affiliated Hospital of Wenzhou Medical University were cultured for GBS.Focusing on the subject to group B streptococci drug susceptibility testing of vaginal secretions and the corresponding samples for analysis.Results A total of 180 GBS isolates were isolated from 5115 women.The prevalence of GBS among 5115 women was 3.52%（180/5115）.All 180 GBS isolates were susceptible to vancomycin,teicoplanin,quinupristin-dalfopristin,tigecycline,ampicillin and penicillin.The susceptibility rates of GBS isolates to nitrofurantoin,moxifloxacin,levofloxacin,ciprofloxacin,erythromycin,clindamycin and tetracycline were 92.22%（166/180）,82.22%（148/180）,81.11%（146/180）,55.00%（99/180）,22.22%（40/180）,10.00%（18/180）,7.78%（14/180）.The p H values of vaginal secretions among77.78 %（140/180） of women with GBS were 〉4.40.Cleanliness degree Ⅲ and Ⅳ were found among54.44 %（98/180） and 17.78 %（32/180） of women.Trichomonas and fungi were detected among 5.56 %（10/180） and 12.22%（22/180）.And 8.89%（16/180）,13.33%（24/180）,98.89%（178/180） and95.56 %（172/180） were positive for clue cells,sialic acid glucoside enzyme,hydrogen peroxide and leukocyte esterase,respectively.Conclusion The prevalence of GBS in the reproductive tracts of women is relatively low.The isolated GBS isolates were susceptible to most antimicrobial agents.Inflammatory condition is found in the reproductive tracts of women with GBS.
