延边地区朝鲜族和汉族学生脂肪细胞因子与不同代谢指标的相关性 被引量:3

Relationship between adipocytokines and metabolic index in Korean-Chinese and Han nationality students of Yanbian area

摘要 目的了解延边地区朝鲜族和汉族学生脂肪细胞因子和不同代谢指标水平及两者的关联性,为早期防控儿童代谢综合征提供参考。方法分别于2011年7月15—18日和2013年7月16—21日,共抽取5 747名延边地区小学生(朝鲜族2 011名,汉族3 736名)作为研究对象,测量调查对象身高、体重、腰围和血压,并测量清晨空腹血清三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、血糖(FBG)、胰岛素(INS)、脂联素、瘦素和脂肪细胞脂肪酸结合蛋白(A-FABP)。结果调整年龄和性别的协方差分析结果显示,延边地区朝鲜族学生脂联素、瘦素和A-FABP平均水平分别为(1.84±0.71)(1.23±0.27)和(4.39±0.67)ng/mL,汉族学生分别为(2.02±0.44)(1.15±0.18)和(4.25±0.96)ng/mL,差异均有统计学意义(P值均<0.01);延边地区朝鲜族学生体质量指数(BMI)、腰围(WC)、收缩压(SBP)、舒张压(DBP)、三酰甘油(TG)、血糖(FBG)和稳态胰岛素评价指数(HOMA-IR)平均水平均高于汉族学生,而HDL-C平均水平低于汉族学生(P值均<0.01)。方差分析结果显示,不同脂联素、瘦素和A-FABP水平朝鲜族和汉族学生代谢指标分布特征差异有统计学意义。多元线性回归分析结果显示,学生脂联素与年龄、性别、民族、BMI,WC和HOMA-IR均密切相关,瘦素与年龄、性别、民族、BMI,WC,SBP,DBP,TG,HDL-C和HOMA-IR均密切相关(P值均<0.05);A-FABP与民族、BMI,WC,TG和HOMA-IR均密切相关(P值均<0.05)。结论朝鲜族和汉族学生脂肪细胞因子和不同代谢指标的分布存在差异;脂肪细胞因子与不同代谢指标的相关性程度有所不同,且存在民族差异。 Objective To investigate relations between adipocytokines and metabolic index in Korean-Chinese and Han nationality students of Yanbian area. Methods A total of 5 747 elementary school students( 2 011 of Korean-Chinese and 3 736 of Han nationality) were selected and investigated during July 2012 to July 2013 in Yanbian area. Height, weight, waist circumference( WC), SBP and DBP was measured. Serum triglyceride( TG), high-density lipoprotein cholesterol( HDL-C), glucose( FBG),insulin( INS), adiponectin( ADP), leptin( LEP) and adipocyte fatty acid binding protein( A-FABP) were measured after 12 hour fasting. Results The levels of ADP, LEP and A-FABP in Korean-Chinese pupils were( 1.84±0.71)( 1.23±0.27)( 4.39±0.67) in Korean-Chinese pupils, and were( 2.02±0.44),( 1.15±0.18) and( 4.25±0.96) in Han nationality pupils, respectively. There was significant difference in levels of adipocytokines between the two nationalities( P〈0. 01). BMI, WC, SBP, DBP, TG, FBG and HOMA-IR in Korean-Chinese pupils were higher than that of Han nationality pupils( P〈0.01), and the level of HDL-C in KoreanChinese pupils was lower than that in Han nationality pupils( P〈0.01) after adjustment for sex and age. ANOVA test showed that students with different levels of ADP, LEP and A-FABP had significant different levels metabolic index. The multivariate regression analysis showed significant relation between ADP and BMI, WC and HOMA-IR( β =-0.074,-0.070,- 0.072), between LEP and BMI, WC, SBP, DBP, TG, HDL- C and HOMA- IR( β = 0. 460,0. 120,- 0. 056,0. 054,0. 066,0. 04,0. 094), and between A-FABP and BMI, WC, TG and HOMA-IR( β = 0.074, 0.096, 0.035 and 0.072). Conclusion Students of different nationalities in Yanbian area had significant level of adipocytokines and metabolic index. Adipocytokines are in close relation with metabolic index.
