内置4F输尿管导管联合200μm光纤钬激光碎石治疗输尿管上段结石 被引量:4

Clinical effects of built-in 4F ureteral catheter combined with 200μm holmium yag laser fiber in treating upper urinary tract stone

摘要 目的 探讨内置4F输尿管导管联合200μm光纤钬激光碎石治疗输尿管上段结石的临床效果.方法 2014年9月至2015年4月本研究应用内置4F输尿管导管联合200μm光纤钬激光碎石治疗46例输尿管上段结石.方法为术中将4F输尿管导管与200μm钬激光光纤一并自输尿管镜插入,碎石的同时,输尿管导管起到排水形成水循环作用,保持视野清晰,降低输尿管内腔压力.结果 本组碎石率为95.6%,手术时间(36.5±4.8) min.住院时间(2.9±0.4)d.术后血尿持续时间(1.8±0.3)d.术中输尿管腔内压力碎石前、结石梗阻解除时、结石梗阻解除后及碎石结束前分别为(25.6-1.2) mmHg、(51.6±4.3)mmHg、(28.7±2.8) mm-Hg、(12.4±3.5) mmHg.无输尿管损伤、发热、大出血.结论 内置4F输尿管导管联合200μm光纤钬激光碎石治疗输尿管上段结石疗效确切,安全可靠,同时经济适用,易于操作. Objectives To explore the clinical effects of built-in 4F ureteral catheter combined with 200μm holmium yag laser fiber in treating upper urinary tract stone.Methods 46 patients with upper urinary tract stone in our hospital from September 2014 to April 2015 were managed with built-in 4F ureteral catheter combined with 200μm holmium yag laser fiber.During the operation,the 4F ureteral catheter played an important role in the water cycling for clear field and lower pressure of ureteral lumen.Results In this study,the lithotripsy rate was 95.66%,the operation time was (36.5 ± 4.8) min,time of hospital stays was (2.9 ± 0.4) d,the duration of blood urine postoperative was (1.8 ± 0.3) d.The ureteral lumen pressure of pre-rubble,stone obstruction lifting,pro-rubble and finished lithotripsy was (25.6 ± 1.2) mmHg,(51.6 ± 4.3) mmHg,(28.7 ± 2.8) mmHg,(12.4 ± 3.5)mmHg,respectively.There was no ureteral injury,fever and bleeding in all cases.Conclusions Built-in 4F ureteral catheter with 200μm holmium yag laser fiber in treating upper ureteral calculi was safe and reliable,affordable and easy to operate.
