苯溴马隆对2型糖尿病合并高尿酸血症男性患者脉搏波传导速度的影响 被引量:5

Effect of benzbromarone on the pulse wave velocity of patients with type 2 diabetes mellitus complicated with hyperuricemia

摘要 目的观察苯溴马隆对2型糖尿病合并高尿酸血症(HUA)男性患者脉搏波传导速度(PwV)的影响。方法选取合并HUA的2型糖尿病男性患者80例,随机分为治疗组(40例)和对照组(40例)。两组在生活方式干预和常规降糖方案的基础上,分别予以苯溴马隆和安慰剂治疗24周。检测患者治疗前后颈一股PwV(cf-PWV)、血压、尿酸、c反应蛋白(CRP)等指标。结果与治疗前比较,苯溴马隆干预24周后,治疗组患者血压、尿酸、CRP水平均明显下降(P〈0.01),cf-PWV明显降低[(11.9±0.6)m/s和(10.2±0.9)m/s,P〈0.01];与对照组比较,治疗组治疗24周后血压、尿酸、CRP明显下降(P均〈0.05),cf-PWV明显降低[(11.5±0.8)m/s和(10.2±0.9)m/s,P〈0.01];尿酸治疗前后变化(△uA)为治疗组cf-PWV变化(Acf-PWV)的独立影响因素(β=0.675,P〈0.01)。结论苯溴马隆降尿酸治疗可以降低2型糖尿病合并HUA男性患者PwV,改善动脉僵硬度。 Objective To investigate the effect of benzbromarone on the pulse wave velocity (PWV) of type 2 diabetic male patients complicated with hyperuricemia (HUA). Methods 80 diabetic men with hyperurieemia were randomly assigned into treatment group(40 patients) and control group(40 patients). Besides of the lifestyle intervention and regular hypoglycemic therapy,the subjects were treated with benzbromarone or placebo respectively for 24 weeks. Carotid-femoral(ef) -PWV as well as blood pres- sure,uric acid,and C-reactive protein (CRP) were measured before and after treatment. Results After treatment,blood pressure, uric acid and CRP significantly decreased when compared with those before treatment in the benzbromarone group( all P 〈 0.01 ). And ef-PWV was significantly reduced after treat- ment [( 11.9 ± 0.6) m/s vs ( 10.2 ±0.9) m/s, P 〈 0.01]. After 24 weeks, blood pressure, uric acid and CRP decreased obviously (all P 〈 0.05 )for patients in benzbromarone group when compared with those in control group. Moreover,ef-PWV decreased significantly in subjects of treatment group E (11.5 ± 0. 8)m/s vs (10.2±0.9)m/s,P 〈0.01 ]. The change of serum uric acid(AUA) was the independent influencing factor for cf-PWV changes ( △cf-PWV) in patients with benzbromarone treatment (β = 0. 675, P 〈 0.01 ). Conclusion Urate-lowering treatment with benzbromarone could reduce the cf-PWV and ameliorate the arterial stiffness for the type 2 diabetic male patients with hyperuricemia.
