摘要
目的探讨Vitek-2Compact AST-GN13对耐碳青霉烯鲍曼不动杆菌左氧氟沙星敏感性测定的准确性。方法收集亚胺培南耐药鲍曼不动杆菌50株,以E-Test法为参考方法,分别用Vitek-2Compact AST-GN13药敏卡和纸片扩散(K-B)法对收集菌株进行左氧氟沙星敏感性检测,并评价3种方法结果的符合性。结果共收集亚胺培南耐药鲍曼不动杆菌50株,E-Test法结果显示共有46株最低抑菌浓度(MIC)>256mg/L,4株MIC=4mg/L;K-B法结果与E-Test法结果相符;Vitek-2Compact AST-GN13检测出现不同结果,其中4株菌株为敏感(≤2mg/L),36株为中介(4mg/L),10株为耐药(≥8mg/L)。K-B法与ETest法标准符合率(CA)为100%,Vitek-2Compact与E-Test法CA为20%,一般错误率为72%,严重错误率为8%。结论Vitek-2Compact AST-GN13检测耐碳青霉烯鲍曼不动杆菌对左氧氟沙星的敏感性时,可能会产生错误结果,日常工作中可采用E-Test与K-B法互为替代方法进行试验。
Objective To investigate the accuracy of the automated system of Vitek-2Compact for the detection of levofloxacin in carbapenem-resistance Acinetobacter baumannii.Methods Fifty strains of carbapenem-resistance Acinetobacter baumannii were collected.The susceptibility of carbapenem-resistance Acinetobacter baumannii to levofloxacin were determined by Vitek-2Compact,disk diffusion method and the E-Test method.E-Test method was used as the reference method for comparison.Results The susceptibility results to levofloxacin in E-Test method showed that 46 stains of resistant(〉256mg/L)and 4strains of intermediary(4mg/L)in fifty strains of carbapenem-resistance Acinetobacter baumannii.The susceptibility of carbapenem-resistance Acinetobacter baumannii to levofloxacin was consistent between disk diffusion method and E-Test method,while different results showed in Vitek-2Compact,which included 4stains of sensitive(≤2mg/L),36 strains of intermediary(4mg/L)and 10 strains of resistant(≥8mg/L).The disk diffusion method gave 100% category agreement(CA)with E-Test.While Vitek-2Compact gave 20% with ETest,72% strains with minor errors,8% strains with very major errors.Conclusion The Vitek-2Compact system might not be suitable for the detection of levofloxacin in carbapenem-resistance Acinetobacter baumannii because of fault result causing,the disk diffusion method could be used as an alternative method for E-Test in the experiment.
