摘要
目的分析维生素D受体(VDR)和P-糖蛋白(P-gp)在胃癌组织中表达的相关性及其与临床病理特征之间的关系。方法 2013年3月—2014年3月收集镇江市康复医疗集团病理中心手术切除标本118例,用免疫组化法检测胃癌组织及癌旁组织中VDR的表达及分布位置,同时检测P-gp的表达,分析两者之间的相关性。结果(1)胃癌组织细胞浆VDR阳性率与癌旁组织无明显差异(U=1.754,P=0.625),但癌组织细胞核VDR阳性率及Pgp阳性率却显著高于癌旁组织(U=10.182,P=0.017)。(2)胃癌组织细胞浆VDR表达强度与P—gp表达强度之间相关性较弱(r=0.243,P<0.01),细胞核VDR表达强度与P-gp表达强度存在正相关(r=0.536,P<0.01);(3)6例免疫印迹证明细胞核VDR阳性的胃癌组织中VDR和P-gp的蛋白表达水平存在正相关(r=0.911,P=0.012)。(4)胃癌组织细胞浆VDR阳性率与分化程度有关(×~2=4.573,P=0.032),细胞核VDR阳性率与肿瘤分化及TNM分期有关(x^2=4.285、5.416,P=0.038、0.020);P-gp阳性率与肿瘤大小、分化程度、浸润深度以及TNM分期有关(x^2=8.807、5.005、14.690、19.508,P=0.003、0.025、0.001、0.000)。结论胃癌组织细胞核VDR表达与P—gp的表达有关。
Objective To explore the expression of VDR and P-gp in the gastric cancer tissue and its relationship with clinical pathological characteristics. Methods 118 cases of surgical resection specimens were collected from March 2014 to March 2013. The expression and localization of VDR and the expression of P-gp were detected in the gastric cancer and its surrounding tissues by immunohistochemistry. The correlation between them was analyzed. Results ( 1 ) Although there was no significant difference between the cytoplasmic positive rate of VDR in carcinomas and adjacent tissues ( U = 1. 754, P = 0. 625 ), the nuclear positive rate of VDR and the positive rate of P-gp in the former were significantly higher than those in the latter( U = 10. 182, P =0.017). (2) The expression of VDR in the nuclei of carcinoma positively correlated with that of P- gp( r = 0. 536, P 〈 0.01 ) ;while there was a weak correlation in the gastric cancer cytoplasm( r =0. 243, P 〈 0. 01 ). (3) Immunoblot demonstrated that there was a positive correlation between the expression levels of VDR and P-gp in the VDR nuclei positive gastric cancer tissues( r =0.911, P =0.012). (4) The cytoplasmic positive rate of VDR was correlated with differen- tiated degree ( X2 = 4. 573, P = 0. 032 ), while the nuclear positive rate of VDR correlated to the differentiation and TNM stages of gastric cancer( X2 =4.285,5. 416, P = 0. 038 ,0. 020) . In addition, the positive rate of P-gp was relevant to the size, differ- entiation, infiltration and TNM stages of gastric cancer( X2 = 8. 807,5. 005,14.690,19. 508, P = 0. 003 ,0. 025 ,0. 001,0.000 ). Conclusion The nuclear expression of VDR in gastric carcinoma is closely related to the expression of P-gp in gastric cancers.
