2013-2015年北京市某社区孕产妇保健管理情况分析

An analysis of maternal health care management situation in a community of Beijing,2013-2015

目的分析社区孕产妇管理存在的问题,为进一步提高社区孕产妇保健管理水平提供理论依据。方法采用回顾性研究方法,对2013年1月-2015年12月在北京市朝阳区某社区卫生服务中心建册的3 208名孕产妇保健管理资料进行统计分析。结果孕产妇平均年龄为30.34岁;剖宫产率为38.34%,且呈逐年下降趋势;孕期早、中、晚建册率分别为61.85%、21.20%和17.05%;有高危因素孕妇占59.35%,高危孕妇率逐年增加,差异有统计学意义(χ2=134.363,P<0.05),高危孕妇剖宫产率高于非高危孕妇,差异有统计学意义(χ2=121.894,P<0.05);68.95%的产妇接受了2次及以上的访视,但访视率逐年降低;系统管理率为98.16%。结论社区孕产妇保健管理应完善孕早期建册模式;加强高危孕产妇管理;降低剖宫产率;完善产后访视,优化服务质量;规范管理制度,加强孕产期保健知识宣传,提高孕产妇管理水平。

Objective To analyze the existing problems of the community maternal and child health management, and to improve level the maternal health care. Methods We reviewed the 3 208 maternal records from 2013 to 2015 in this community health service center. Results The mean age of the 3 208 pregnant women was 30. 34. The rate of cesarean section was 38.34% and decreased gradually. The establishment of archives on maternal health were 61.85% （first-trimester）, 21.20% （second trimester）, and 17. 05% （last trimester）. High-risk pregnant women was 59. 35%, and its rate increased gradually in 3 years（x^2 = 134. 363, P 〈 0.05 ）. The delivery way between high-risk and normal pregnant women was statistically different （x^2= 121. 894,P 〈 0.05）. 68.95% puerpera had received at least twice postpartum follow-up, the rate decreased gradually in 3 years. The coverage of systemic maternal management was 98.16%. Conclusion We should improve the mode of heahhcare registry of early pregnancy, strengthen the management of high-risk pregnant women, reduce the rate of cesarean section, and encourage postpartum follow-up and optimize supervision services. By standardizing management system, the level of community maternal health management would be improved, and the knowledge promotion of maternal health care be enhanced.
