子痫前期外周血中间型单核细胞HLA-DR的表达及其临床意义 被引量:4

Expression of HLA-DR on peripheral blood intermediate monocytes and its functional implication in pre-eclampsia

摘要 目的:通过检测子痫前期患者外周血中间型单核细胞HLA-DR表达并分析其与临床指标的相关性,探讨中间型单核细胞HLA-DR在子痫前期疾病过程中的作用。方法:采集22例子痫前期患者(子痫前期组)和23例健康孕妇(对照组)肘前静脉血4 m L,肝素抗凝。流式细胞术检测中间型单核细胞HLA-DR表达和Th17细胞阳性率;液相芯片检测血清中肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6、IL-17和IL-10浓度;分析子痫前期患者中间型单核细胞HLA-DR阳性频率与血清细胞因子浓度、Th17细胞阳性率的相关性。结果:子痫前期组中间型单核细胞HLA-DR阳性细胞率为(29.4±8.4)%,比对照组[(20.1±3.8)%]明显增高(P<0.01);平均荧光强度(MFI)为31.5±5.9,亦明显高于对照组(26.4±4.3,P<0.01)。与对照组比较,子痫前期患者血清中TNF-α、IL-6和IL-17浓度显著升高,IL-10浓度明显降低,差异均有统计学意义(P<0.05);子痫前期患者中间型单核细胞HLA-DR阳性细胞率与Th17细胞阳性率、血清IL-17浓度均呈正相关(r=0.468、r=0.473,P<0.05)。结论:子痫前期患者外周血中间型单核细胞HLA-DR表达明显增加,参与子痫前期的疾病过程。 Objective: To investigate the role of HLA-DR on peripheral blood intermediate monocyte in the pathogenesis of pre-eclampsia, and the expression of HLA-DR on peripheral blood monocytes from patients with pre-eclampsia and their correlation with the clinical biomarkers. Methods: Twenty-two patients with established pre-eclampsia patients and 23 cases of healthy pregnant women (HP controls) were selected in this study. Four mL of fresh venous blood was collected into a tube containing heparin after all participants gave informed written consent. The expression of HLA-DR on intermediate monocyte and the frequency of Th17 were evaluated with the methods of flow cytometry. The concentrations of IL-6, IL-17, IL-10 and TNF-α in serum were analyzed with the method of Luminex liquichip platform. The correlation between the frequencies of HLA-DR + intermediate monocytes and serum cytokine levels were also evaluated. Results: Compared to HP controls, the percentage of HLA-DR + intermediate monocyte was marked- ly higher in pre-eclampsia patients [ (29.4 ± 8.4 ) % vs (20.1 ± 3.8 ) % ,P 〈 0.01 ], as well as the mean fluorescence intension of HLA-DR( 31.5 ± 5.9 vs 26.4 ± 4.3, P 〈 0.01 ). The serum concentrations of IL-6,IL-17 and TNF-α in pre-eclampsia patients were significantly higher than that of HP controls(P 〈 0.05 ) , while the level of IL-10 were significantly lower than that of HP controls ( P 〈 0. 05 ). In addition, a positive correlation has been found between the percentage of HLA-DR + intermediate monocytes and the serum levels of IL-17 and the frequency of Thl7 in pre-eclampsia patients ( r = 0. 468, r = 0. 473, P 〈 0.05). Conclusion: Intermediate monocytes from patients with pre-eclampsia exhibited increased HLA- DR expression, which might play a crucial role in the pathogenesis of preeclampsia.
