柴胡加龙骨牡蛎汤加减方联合心理干预治疗肿瘤后抑郁临床研究

Clinical Study on Combination of Modification of Chaihu Jia Longgu Muli Decoction with Psychological Intervention in Treatment of Post Tumor Depression

目的观察柴胡加龙骨牡蛎汤加减方联合心理干预治疗肿瘤后抑郁的临床疗效。方法将122例肿瘤后抑郁患者采用简单数字表随机方法分为治疗组和对照组各61例。治疗组予柴胡加龙骨牡蛎汤加减方口服,同时给予心理干预;对照组予草酸艾司西酞普兰片口服。连续治疗6周。观察2组患者治疗前后汉密顿抑郁量表(HAMD)评分,评价临床疗效,采用SF-36量表评价治疗前后生活质量,并观察不良反应。结果治疗后治疗组HAMD评分低于对照组(P<0.05);治疗组总有效率为88.52%(54/61),对照组总有效率为72.13%(44/61),组间比较差异有统计学意义(P<0.05);治疗后生活质量评分优于对照组(P<0.05),不良反应发生率低于对照组(P<0.05)。结论柴胡加龙骨牡蛎汤加减方联合心理干预治疗肿瘤后抑郁能明显改善患者的抑郁状态,提高其生活质量,疗效优于对照组,且无明显不良反应。

Objective To observe the clinical efficacy of the combination of modification of Chaihu Jia Longgu Muli Decoction with psychological intervention in the treatment of post tumor depression. Methods Totally 122 cases of post tumor depression were divided into treatment group and control group by simple numeration table random method, with 61 cases in each group. The treatment group was given modification of Chaihu Jia Longgu Muli Decoction orally, at the same time psychological intervention was given. The control group was given escitalopram oxalate tablets for 6 weeks. HAMD scale scores of the two groups were observed before and after treatment to evaluate the clinical efficacy. Changes in life quality of two groups were scored with SF-36 scale. The adverse reactions were also observed. Results After treatment, the HAMD score of treatment group was lower than control group（P〈0.05）. The total effective rate was 88.52%（54/61） in the treatment group and 72.13%（44/61） in the control group, with statistical significance（P〈0.05）. The scores of life quality of treatment group were higher than control group（P〈0.05）, and the incidence of adverse reactions was lower than control group（P〈0.05）. Conclusion Combination of modification of Chaihu Jia Longgu Muli Decoction with psychological intervention in the treatment of post tumor depression can significantly improve the depressive state and life quality of the patients with post tumor depression, which is better than the control group, without significant adverse reactions.
