复方四黄液联合闭式引流技术治疗感染化脓创面的临床研究

A study on treating infective purulent wound with the Fufang Sihuang liquid plus sealing drainage technique

目的:探讨复方四黄液联合闭式引流技术对感染化脓创面患者创面修复的影响。方法:病例来源于2014年1月-2015年7月来我院门诊就医的化脓感染创面患者90例,按随机数字表法分为观察组和对照组,对应数字偶数为观察组,奇数为对照组,每组45例。对照组患者单独应用封闭负压引流术处理创口,观察组在对照组基础上联合复方四黄液治疗,比较两组患者的恢复情况、疼痛程度及临床疗效,并测定治疗后患者创面渗出液中IL-6、TNF-α的表达及静脉血检测WBC计数、CRP含量。结果:观察组总有效率为91.11%(41/45),明显高于对照组的77.78%(35/45),差异具有统计学意义(P<0.05);治疗后观察组患者的红肿消退时间、肉芽生长时间、上皮组织生长时间、疼痛程度评分均明显短(或少)于对照组,差异具有统计学意义(P<0.05);观察组患者治疗后的IL-6、TNF-α的表达、CRP含量、WBC计数均明显低于对照组,差异均具有统计学意义(P<0.05)。结论:复方四黄液联合闭式引流技术可有效地促进感染化脓创面肉芽组织的生长及修复,此外还可下调IL-6、TNF-α的表达,抑制炎症反应,促进患者创面愈合,提高临床治疗效果,值得在临床推广应用。

Objective： To explore effcacy of the Fufang Sihuang liquid plus sealing drainage technique on infective purulent wound. Methods： 90 cases were divided into the observation group and the control group according to random number table method, 45 cases in each group. The control group was given sealing drainage technique only; the observation group was treated with the Fufang Sihuang liquid more. The pain degree, recovery condition and clinical effects in groups were compared; the IL-6 and TNF-αin the wound exudate, the WBC and CRP in the venous blood were checked. Results： The total effciency in the observation group was 91.11%, and signiifcantly higher than 77.78% in the control group, with a statistically signiifcant difference （P〈0.05）. The redness fade time, granulation growth time and epithelial tissue growth time were signiifcantly shorter; pain score was much less in the observation group. The IL-6 and TNF-α, the WBC and CRP were signiifcantly lower in the observation group, with a statistically signiifcant difference （P〈0.05）. Conclusion： The Fufang Sihuang liquid plus sealing drainage technique can effectively promote growth and repair of granulation tissue for heal wound, and reduce the IL-6 and TNF-α, control the inlfammatory response, improve the effcacy, and is worthy of popularization.
