经椎弓根椎体截骨技术在胸腰椎后凸畸形矫正中的应用

Application of pedicle vertebral osteotomy in thoracolumbar protrusion deformity correction

目的 探讨经椎弓根椎体截骨技术在胸腰椎后凸畸形矫正中的应用价值。方法 将2013年5月-2015年1月因胸腰椎后凸畸形于我院行经椎弓根椎体截骨手术治疗的32例患者纳入研究,观察患者手术前及手术后6个月时疼痛[采用视觉模拟量表（VAS）]评分,矢状面Cobb角,Oswestry功能障碍指数（ODI）的改善情况。结果 治疗前,患者VAS评分为（7.64±1.26）分,矢状面Cobb角为（45.79±12.43）°,ODI水平为（58.67±7.50）%;治疗后VAS评分为（3.25±0.78）分,矢状面Cobb角为（10.52±6.85）°,ODI水平为（26.98±5.40）%,均较治疗前降低,差异有统计学意义（P〈0.01）。结论 经椎弓根椎体截骨技术在胸腰椎后凸畸形矫正中具有重要的应用价值,可更好地纠正畸形,缓解疼痛,值得推广。

Objective To explore the application value of pedicle vertebral osteotomy in thoracolumbar protrusion defor- mity correction.Methods 32 patients treated with pedicle vertebral osteotomy for thoracolumbar protrusion deformity from May 2013 to January 2015 were selected.The pain score [using visual analogue scale （VAS）] before and 6 months after operation,sagittal Cobb angle and the improvement situation of Oswestry disability index （ODI） of patients were observed.Results Before treatment,the VAS score,sagittal Cobb angle and ODI index level of patients were respectively （7.64±1.26） points,（45.79±12.43）° and （58.67±7.50）%;after treatment,the VAS score,sagittal Cobb angle and ODI index level of patients were respectively （3.25±0.78） points,（10.52±6.85）°and （26.98±5.40）%,all of that decreased than those before treatment,and the difference was statistical significance （P〈0.01）.Conelusion Pedicle vertebral osteotomy has great application value in thoraeolumbar protrusion deformity eorreetion,whieh can correct malformation better, alleviate pain and is worth to popularize.
