摘要
目的探讨应用胰岛素外敷治疗糖尿病足时胰岛素剂量及护理。方法糖尿病足患者240例随机分为4组,每组60人,其中对照组和三个治疗组。对照组则在常规换药治疗基础上未加胰岛素进行外敷护理。治疗组在常规换药治疗基础上,分别对三组患者给予不同剂量胰岛素外敷。每日早、晚分别换药1次,10天后评价记录创面愈合情况,40天结束评价。应用SPSS19.0软件,所获数据采用方差分析、t检验和X^2检验。结果治疗前各组间年龄、性别、病程、空腹血糖、餐后2 h血糖、创面大小比较,P>0.05,无显著性差异。对照组有效44例,占73.33%;治疗1组有效52例,占86.67%;治疗2组有效58例,占96.67%,;治疗3组有效52例,占93.33%。有效率对照组与治疗2组比较,X^2=8.9571,P<0.005,有非常显著性差异。治疗2组与治疗3组比较,X^2=3.9272,P<0.05,有显著性差异。对照组创口平局愈合时间34.27±2.58天;三个治疗组平均创口愈合时间20.43±1.71天。治疗1组平均创口愈合时间20.56±1.57天,治疗2组平均创口愈合时间17.33±1.74天,治疗3组平均创口愈合时间23.41±1.82天。治疗各组与对照组比较,t=26.643~42.165,P<0.0001,有非常显著性差异。三个治疗组合并与对照组比较,t=36.462,P<0.0001,有非常显著性差异。治疗组内比较,治疗1组与治疗2组、治疗3组及治疗2组与治疗三组比较,t=8.6466~17.442,P均<0.0001,有非常显著性差异。结论在糖尿病足治疗中外敷时加用胰岛素8U效果显著,值得推广。
Objective To study the application of insulin topical insulin doses and nursing treatment of diabetic foot. Methods 240 patients with diabetic foot were randomly divided into four groups, each group of 60 people, three control group and treatment group. Control group on the basis of conventional dressing therapy for topical care without insulin. Based on conventional dressing therapy, the treatment group were given different doses of insulin in patients with three groups of gels. Daily treatment 1 times respectively in the morning and at night, 10 days after the evaluation records of wound healing, 40 days end. SPSS19.0 application software, the data obtained using analysis of variance, t test and χ2 test. Results Before treatment between groups of age, gender, duration, fasting blood sugar, 2 h postprandial blood glucose, and the size of the wound, P〉0.05, no significant difference. The control effective 44 cases, accounted for 73.33%. Treatment group 1 effective 52 cases (86.67%). Treat group of 58 cases effectively, 2 (96.67%), 3 set of effective treatment of 52 cases, accounting for 93.33%. Efficient control group and treatment group 2 χ2 = 8.9571, P 〈 0.005. There is a very significant difference. 2 group and 3 treatment group to compare the χ2 = 3.9272, P〈0.05. There is significant difference. Control group wound healing time draw 34.27 ±2.58 days. Three treatment group the average wound healing time 20.43 ±1.71 days. Group 1 was 20.56 ±1.57 days, the average wound healing time for 2 set of the average wound healing time 17.33 ±1.74 days, treat 3 groups the average wound healing time 23.41± 1.82 days. The treatment group compared with control group t = 26.643 - 42.165, P〈 0.0001. There is a very significant difference. Three combination treatment and com pared-with control group, t = 36.462, P〈0.0001. There is a very significant difference. In the treatment group, treatment 1 and treatment 2 group, 3 group and treatment group and treatment group 2, t = 8.6466 - 8.6466, P〈0.0001. There is a very significant difference. Conclusion When the external treatment in diabetic foot treatment with insulin 8 u effect is remarkable, is worth promoting.
