尿激酶与不同剂量立普妥在急性心肌梗死溶栓中的效果比较

The effect comparation of urokinase and different doses lipitor in the thrombolysis treatment of acute myocardial infarction

目的探讨尿激酶与不同剂量立普妥在急性心肌梗死溶栓中的效果。方法选择我院接受治疗的急性心肌梗死患者82例为研究对象,分为低剂量组和高剂量组,每组41例,所有患者均采用尿激酶进行溶栓治疗,低剂量组患者溶栓前给予立普妥20 mg口服,高剂量组患者溶栓前使用剂量为40 mg/d,比较两组患者血管再通率、胸痛评分以及不良反应发生率。结果高剂量组血管再通率为78.05%,明显高于低剂量组的58.54%,两组比较差异具有统计学意义(P<0.05)。两组患者治疗前胸痛程度评分比较差异无统计学意义(P>0.05),治疗1周后,高剂量组患者评分低于低剂量组,两组比较差异具有统计学意义(P<0.05)。两组不良发应发生率比较差异无统计学意义(P>0.05)。结论在使用尿激酶进行溶栓治疗时,与小剂量立普妥相比,大剂量立普妥能够提高血管再通率,降低胸痛程度评分,能更有效地改善急性心肌梗死患者预后。

Objective To investigate the effect of urokinase and different doses lipitor in the thrombolysis treatment of acute myocardial infarction. Methods Eighty-two patients with acute myocardial infarction treated in our hospital were selected and divided into low dose group （41 cases） and high dose group （41 cases）, all patients were given the urine kinase thrombolytic therapy; before thrombolysis, the low dose group was given 20 mg lipitor orally, while the high dose group used a dose of 40 mg. The vesse repass rate, pain score and adverse reaction incidence of the two groups were compared. Results The vessel repass rate of the high dose group （78.05%） was significantly higher than that of the low dose group （58.54%）, the difference had statistical significance （P〈0.05）. The chest pain score between the two groups before treatment had no significant difference（P〈0.05）, after one week treatment, the score of the high-dose group was lower than that of the low-dose group, the difference had statistical significance （P〈0.05）. The difference of adverse reaction incidence had no statistical significance （P〈0.05）. Conclusion For thrombolytic therapy, compared with low-dose lipitor, large doses lipitor can improve vessel repass rate, reduce chest pain score, and effectively improve the prognosis of patients with acute myocardial infarction.
