摘要
目的 探讨口腔鳞状细胞癌(简称鳞癌)“瘤芽”的自噬活性变化及其与颈淋巴转移的关系。方法 应用免疫组化染色检测34例口腔鳞癌组织及“瘤芽”中自噬启动基因Beclin1和自噬标记蛋白LC3的表达,结合临床病理资料分析其表达与颈淋巴转移的关系。数据分析采用SPSS 19.0统计软件,计数资料之间比较采用卡方检验。结果 Beclin1和LC3在癌旁非肿瘤上皮、癌组织和“瘤芽”中的表达呈递减趋势,组间差异有统计学意义(P〈0.05);有颈淋巴结转移和无颈淋巴结转移口腔鳞癌“瘤芽”的LC3表达差异有统计学意义(P=0.008),“瘤芽”中Beclin1和LC3表达水平较低的患者颈淋巴转移率较高;在TNM分期中,Ⅲ~Ⅳ期口腔鳞癌“瘤芽”的LC3表达水平显著低于Ⅰ~Ⅱ期口腔鳞癌“瘤芽”(P=0.002);但不同分化程度口腔鳞癌“瘤芽”的Beclin1和LC3表达差异无统计学意义(P〉0.05)。结论 口腔鳞癌“瘤芽”自噬活性低于癌组织和癌旁非肿瘤上皮,“瘤芽”自噬水平越低,越容易发生颈淋巴转移,进一步证明降低自噬可以促进口腔鳞癌的颈淋巴转移。
Objective To evaluate the expression and significance of Beclin1 and LC3 in tumor budding of oral squamous cell carcinoma. Methods Immunohistochemical and H.E staining were used to detect the Beclin1 and LC3 expression and assess tumor budding in 34 patients with oral squamous cell carcinoma. The association among Beclin1,LC3,tumor budding and patient clinical features were analyzed with Chi?square test. Data were analyzed by Chi square test using SPSS 19.0 statistical software. Results Decreased expression of Beclin1 and LC3 was confirmed in cancer center when compared with adjacent non?cancerous tissue. Similar results were also observed in tumor budding cells when compared with cancer center. The decreased expression of LC3 in tumor budding was associated with cervical lymph node metastasis and clinical stage in patients with oral squamous cell carcinoma(P<0.05). Conclusion The activity of autophagy was decreased in tumor budding,which was associated with cervical lymph node metastasis in oral squamous cell carcinoma.
