后路钉棒系统内固定结合针灸治疗胸腰椎骨折合并脊髓损伤

Posterior screw-rod system internal fixation combined with acupuncture in the treatment of thoracolumbar fracture complicated with spinal cord injury

目的探讨后路钉棒系统内固定结合针灸治疗胸腰椎骨折合并脊髓损伤的方法和疗效。方法 52例胸腰椎骨折合并脊髓损伤患者,随机分为对照组25例和治疗组27例,对照组采用单纯后路钉棒系统内固定治疗,治疗组采用后路钉棒系统内固定结合针灸治疗。比较两组的治疗效果。结果经过9~26个月的随访,平均随访（12.7±4.5）个月,两组患者骨折椎体前后缘高度恢复满意,脊柱生理曲度恢复良好。术后治疗组脊髓功能Frankel分级优于对照组（P〈0.05）。结论后路钉棒系统内固定结合针灸治疗胸腰椎骨折合并脊髓损伤可获得良好的脊柱稳定性,明显改善损伤脊髓功能,值得临床推广应用。

Objective To investigate measure and curative effect by posterior screw-rod system internal fixation combined with acupuncture in the treatment of thoracolumbar fracture complicated with spinal cord injury. Methods A total of 52 thoracolumbar fracture complicated with spinal cord injury patients were randomly divided into control group with 25 cases and treatment group with 27 cases. The control group received posterior screw-rod system internal fixation alone for treatment, and the treatment group received posterior screw-rod system internal fixation combined with acupuncture for treatment. Curative effects were compared between the two groups. Results After 9~26 months of followed-up, with mean time as（12.7±4.5） months, both groups had satisfactory recovery of vertebral anterior and posterior border height and physiological vertebral curvature. After operation, the treatment group had better Frankel classification of spinal function than the control group（P〈0.05）. Conclusion Combination of posterior screw-rod system internal fixation and acupuncture provides remarkable spinal stability in the treatment of thoracolumbar fracture complicated with spinal cord injury. This method can obviously improve injured spinal function, and it is worth clinical promotion and application.
