P53在结节性甲状腺肿合并甲状腺癌中的表达及其相关性研究

P53 Expression in Nodular Goiter with Thyroid Cancer and Their Correlation

目的探讨结节性甲状腺肿合并甲状腺癌患者中P53的表达及其临床意义。方法对70例结节性甲状腺肿合并甲状腺癌标本,采用免疫组织化学SP法检测P53表达,并与同期77例单纯结节性甲状腺肿组织的P53表达情况进行对比分析。结果在甲状腺肿合并甲状腺癌中P53阳性表达率为47.14%(33/70),单纯结节性甲状腺肿中其阳性表达率为10.39%(8/77),两组差异有显著性(P<0.05)。在单纯结节性甲状腺肿组及甲状腺肿合并甲状腺癌组中,年龄<50岁与>50岁者P53阳性率比较均无显著性差异(P>0.05)。甲状腺肿合并甲状腺癌70例患者中肿瘤直径<3cm及>3 cm的患者P53阳性率分别为39.02%、58.62%,有淋巴结转移与无淋巴结转移患者P53阳性率分别为59.26%、39.53%,两者均无显著差异性(P>0.05)。结论 P53表达在结节性甲状腺肿患者的甲状腺癌发生发展过程中具有特异性作用,对于结节性甲状腺肿合并甲状腺癌的诊疗具有重要意义。

Objective To explore the expression of p53 and clinical significance in the nodular goiter complicated with thyroid carcinoma.Methods Selected 70 cases of nodular goiter with thyroid cancer,and use immunohistochemical SP method to detect P53 expression.And it was analyzed and compared with P53 expression of 77 cases of nodular goiter.Results The p53 mRNA positive expression rate in goiter complicated with thyroid carcinoma group was 47.14%（33/70）,while the positive rate in the pure nodular goiter group was 10.39%（8/77）,and there was significant differences between the 2 groups（P〈0.05）.In pure nodular goiter group and thyroid goiter merger carcinoma group,the difference of P53 positive rate between patients less than50 and over 50 was not significant（P〉0.05）.Among 70 cases of patients with goiter with thyroid carcinoma,the P53 positive rates of patients with diameter of tumor less than or equal to 3 cent and more than 3 cent were 39.02% and 58.62% respectively.The P53 positive rates of patients with lymph node metastasis and without were 59.26% and 39.53% respectively,the differences were no significant（P〉0.05）.Conclusion The p53 expression of nodular goiter with thyroid carcinoma has a specific role in the process of disease progression,which is of great significance for the diagnosis of nodular goiter with thyroid cancer.
