摘要
目的探讨青年乳腺癌患者的病理特征及预后的相关影响因素。方法选取35岁以下(包括35岁)的乳腺癌患者60例,另外随机选择同期收治的年龄在35岁以上的中老年患者60例作为对照。回顾性分析患者的临床病理资料和影响预后的相关因素。结果 2组患者的组织学类型和组织学等级无统计学差异(P>0.05);青年组患者ER和PR的阳性率小于中老年组患者,HER-2的阳性率大于中老年组患者,差异有统计学意义(P<0.05)。单因素分析结果显示年龄、肿瘤大小、组织学类型、组织学分级、ER阳性、PR阳性、HER-2阳性、淋巴结转移和是否采用内分泌治疗是影响青年乳腺癌患者预后的因素。多因素分析结果显示年龄、肿瘤大小、组织学类型、组织学分级、ER、PR、HER-2、淋巴结转移是影响青年乳腺癌患者预后的独立危险因素。结论青年乳腺癌患者出现ER、PR阴性和HER-2阳性的概率较高。预后受年龄、肿瘤大小、组织学类型、组织学分级、ER、PR、HER-2、淋巴结转移等多种因素影响。年轻女性应当注重定期体检,做到早发现、早诊断、早治疗。
Objective To investigate the pathological characteristics of young breast cancer and the related factors affecting the prognosis.Methods 60 breast cancer patients ≤35 were selected,and 60 breast cancer patients over 35 was the control.Clinical data of patients and related factors affecting prognosis were retrospectively analyzed.Results There was no significant difference between the 2 groups(P〉0.05) in histological type and histological grade.The positive rates of PR and ER in the young group were lower than those of the elderly patients.The positive rate of HER-2 was higher than that of the elderly group,and the difference was statistically significant(P〈0.05).Single factor analysis showed that age,tumor size,histological type,histological grade,ER,PR,HER-2,lymph node metastasis and the use of endocrine therapy were the influence factors for prognosis of young breast cancer patients.Multivariate analysis showed that age,tumor size,histological type,histological grade,PR,ER,HER-2,lymph node metastasis were independent risk factors for prognosis in young patients with breast cancer.Conclusion Young breast cancer patients have a high probability of ER,PR negative and HER-2 positive rate.The prognosis is influenced by age,tumor size,histological type,histological grade,ER,PR,HER-2 and lymph node metastasis.Young women should pay attention to regular physical examination so as to achieve early detection,early diagnosis and early treatment.
