从“升降相因”角度探讨咳嗽的发病机制与治疗

升降的基础是阴阳之升降,升降相因是指升降运动在人体内各脏腑及脏腑之间的相互协调,脏腑功能的核心是升降运动。咳嗽总由肺气上逆所致,咳嗽不离肺而非独肺,肝、肾、脾胃、心的升降运动异常均会导致咳嗽的发生,如肝升太过、肾不纳气、脾胃升降失常、心火不降,故治疗咳嗽时当辨清脏腑阴阳,从整体出发着重调畅脏腑气机升降运动。故从升降相因理论角度探讨咳嗽的发病机制,为咳嗽的辨证治疗和临床用药提供重要的理论依据。

Yin-yang ascending and descending movement is the foundation of ascending and descending movement.The coordinate of ascending and descending is the harmonization of ascending and descending in viscera physiological activities and each other. The core function of viscera is lifting movement. Not only adverse rising of qi but also disorders of qi ascending and descending movement of liver、kidney、heart、spleen and stomach such as excessive liver-Qi、kidney failing to receive Qi、spleen-stomach disharmony、excessive heart-fire lead to the occurrence of cough. The treatment of cough should be distinguished between viscera Yin and Yang from the whole. The regulation of qi ascending and descending movement of viscera is the critical principle for the treatment of cough. This thesis tries to focus on the mechanism of cough from the point of ascending-descending theory in order to provide important theoretical basis for syndrome differentiation and treatment of cough.
