膝关节镜联合玻璃酸钠治疗膝骨关节炎95例疗效分析 被引量:12

摘要 目的探讨膝关节镜联合玻璃酸钠治疗膝关节骨性关节炎的疗效。方法选取95例膝关节骨性关节炎患者,按入院时间段分为两组,对照组(46例)单纯采用膝关节镜下清理术治疗,观察组(49例)行膝关节镜下清理术联合关节腔内注射玻璃酸钠治疗,观察并比较两组治疗前后膝关节Lysholm评分、疗效、血清白介素-1β(IL-1β)和肿瘤坏死因子(TNF-α)水平及随访6个月期间不良反应情况。结果术后两组膝关节评分升高(P<0.05),且观察组评分高于对照组(P<0.05)。观察组治疗的优良率显著高于对照组(P<0.05)。术后3个月,两组炎性因子明显降低,观察组降低程度大于对照组(P<0.05)。随访6个月期间,两组不良反应事件较少,均无明显并发症。结论膝关节镜下清理术联合关节腔内注射玻璃酸钠对膝关节骨性关节炎具有较好的治疗效果,可以有效提高患者膝关节功能,降低关节内炎性介质的表达水平,无明显并发症,值得临床推广使用。 Objective To explore the efficacy of knee arthroscopy combined with sodium hyaluronate in the treatment of knee osteoarthritis, Methods A total of 95 patients with knee osteoarthritis were selected and divided into two groups according to the date of admission. The control group (n = 46) was treated with knee arthroscopy only, and the observation group (n = 49) was treated with knee arthroscopy combined with sodium hyaluronate. The Lyshohn score, efficacy, the levels of IL-1β and TNF-α and the incidence of adverse reactions within the six-month follow-up visit in the two groups were observed and compared. Results The knee scores in the two groups after the treatment increased (P 〈 0.05); the scores in the observation group were higher than those in the control group (P 〈 0.05). The fineness rate in the observation group was much higher than that in the control group (P 〈 0.05). Three months after the operation, the inflammatory factors in the two groups decreased greatly, and the reduction in the observation group was greater than that in the observation group (P 〈 0.05). During the six-month follow-up visit, the adverse reactions in the two groups were very few, and there were no obvious complications. Conclusion Knee arthroscopy combined with sodium hyaluronate has good effects on the treatment of knee osteoarthritis, can improve the knee function and lower the expression of inflammatory mediators in the joint without obvious complications. Therefore, it is worthy of clinical popularization and application.
