摘要
目的对比分析Cook子宫颈扩张球囊和地诺前列酮对晚期妊娠促宫颈成熟的效果。方法选择有引产指征的180例产妇,其中100例用Cook子宫颈扩张球囊促宫颈成熟为观察组,80例用地诺前列酮促宫颈成熟为对照组。在治疗前后进行宫颈Bishop评分,比较两组的临产时间、总产程时间、第1、2产程时间和剖宫产率,以及并发症的发生情况。结果观察组促宫颈成熟有效率为97.00%,明显高于对照组的86.25%(P<0.05);观察组的临产时间、总产程时间、第1、2产程时间和剖宫产率均明显低于对照组(P<0.05);观察组强直宫缩、胃肠道反应、急产、胎心异常、产后出血、新生儿窒息、羊水污染等并发症发生率为7.00%,明显低于对照组的16.25%(P<0.05)。结论 Cook子宫颈扩张球囊用于晚期妊娠促宫颈成熟效果好,能明显降低剖宫产率,且并发症较少,是一种有效的促宫颈成熟方法。
Objective To compare the effects of Cook cervical dilatation balloon and dinoprostone on the cervical ripening in late pregnancy. Methods A total of 180 puerperas with induced labor indication were selected and divided into an observation group (n = 100) and a control group (n = 80). Cook cervical dilatation balloon was applied to the cervical ripening in the observation group and dinoprostone to the control group. The cervical Bishop score before and after the treatment was evaluated; the time of in labor, total duration of labor, the duration of the first and second stages of labor, cesarean section rate and the incidence of complications in the two groups were compared. Results The response rate of cervical ripening in the observation group was 97.00%, which was significantly higher than that (86.25%) in the control group (P 〈 0.05); the time of in labor, total duration of labor, the duration of the first and second stages of labor and cesarean section rate in the observation group were much shorter or lower than those in the control group (P 〈 0.05). The incidence of such complications as tetanic contraction, gastrointestinal reaction, precipitate labor, abnormal fetal heart, postpartum hemorrhage, neonatal asphyxia and amniotic fluid pollution in the observation group was 7.00%, which was significantly lower than that (16.25%) in the control group (P 〈 0.05). Conclusion Cook cervical dilation balloon has good effects on the cervical ripening in late pregnancy, can significantly lower the rate of cesarean section and the incidence of complications. Therefore, it is an effective method for cervical ripening.
