关节镜下骨赘清理治疗膝关节骨性关节炎的效果观察

Observation of effect of osteophyte cleaning under arthroscope for patients with knee osteoarthritis

目的探讨关节镜下有限清理联合骨赘清理治疗膝关节骨性关节炎的临床疗效.方法选取2014年3 月-2015年2月收治的89例膝关节骨性关节炎患者,随机分为研究组和对照组,对照组行关节镜下有限清理术,研究组在对照组基础上联合骨赘清理,术后随访6个月,比较两组患者膝关节功能、膝关节Lyshlom评分及膝关节疼痛VAS评分.结果研究组患者膝关节功能优良率（98.15%）高于对照组（82.86%）,比较差异有统计学意义（P〈 0. 05）.两组患者术前Lyshlom评分和膝关节疼痛VAS评分比较无统计学意义（P〉0.05）;术后6 个月研究组患者 Lyshlom 评分高于对照组[（87.9±3.5）分 vs（76.1±3.5）分], VAS 评分低于对照组[（1.7±0.8）分 vs（3.5±1.0）分],组间比较差异有统计学意义（P〈0.001）.结论关节镜有限清理联合骨赘清理治疗膝关节骨性关节炎可有效提高膝关节功能,降低术后疼痛,其效果更为显著.

Objective To investigate the clinical efficacy of limited debridement combined with osteophyte cleaning un-der arthroscope in the treatment of knee osteoarthritis. Methods 89 cases of knee osteoarthritis admitted from March,2014 to February,2015 were enrolled in the study and randomly divided into study group and control group.The control group were given limited arthroscopic debridement,and based on which the research group were added osteophyte cleaning under arthro- scope.All patients were given 6 months of follow-up visit.And then, knee joint function, knee joint Lyshlom score and knee pain VAS score were compared between the two groups. Results The excellent rate of knee joint function in the study group （98.15%） was higher than that （82.86%） of the control group, so difference was statistically significant（ P 〈0.05）.Differ- ence of Lyshlom score and knee pain VAS score before operation in the two groups was not statistically significant（P 〉0.05）. 6 months after operation,Lyshlom score of the study group was higher than that of the control group[（87.9±3.5） vs （76.1± 3.5） points],and VAS score was lower than that of the control group [（1.7±0.8） vs （3.5±1.0） points], so difference was statistically significant（P 〈0.001）.Conclusion Arthroscopic limited debridement combined with osteophyte cleaning in the treatment of knee osteoarthritis can effectively improve the knee joint function, alleviate postoperative pain, and the effect is more significant.
