肿瘤患者PICC导管相关血栓形成危险因素的研究

The risk factors of thrombosis associated with Peripherally Inserted Central Catheters among cancer patients

目的：分析肿瘤患者PICC导管相关血栓形成的危险因素。方法：收集广州市两所三级甲等综合医院2014年1月1日至12月31日留置PICC肿瘤患者一般人口学资料、置管前一周内血样检查结果、病史资料、置管资料及置管3个月内并发症发生情况，采用Logistic同归分析筛选PICC导管相关血栓形成危险因素。结果：确诊癌症7个月及以上、肿瘤转移、有置管期间放疗史、置管目的为输注脂肪乳／甘露醇等是肿瘤患者PICC导管相关血栓形成的危险因素，OR值及其95％CI分别为1．832（1．092-3．386）、2．358（1．113-4，996）、4．348（1．902-9．941）、3．442（1．044-11．341）。结论：肿瘤患者PICC导管相关血栓形成的危险因素包括确诊癌症7个月及以上、肿瘤转移、有置管期间放疗史、置管目的为输注脂肪乳／甘露醇等。

Objective： To determine the risk factors of thrombosis associated with Peripherally Inserted Central Catheters among cancer patients. Methods： The research was conducted in two tertiary Grade A general hospitals in Guangzhou. It was a retrospective study from January to December, 2014. We recruited the eligible patients and investigated their demographic data, medical history and complications within 3 months of catherization, and then used multiple logistic regression analysis to determine PICC related thrombosis risk factors among cancer patients. Results： Risk factors positively associated with thrombosis were diagnosed cancer for more than seven month, metastases, history of radiation, and fat emulsion/mannitol infusion, and the value of OR and 95% CI were 1.832 （1.092-3.386）, 2.358 （1.113- 4.996）, 4.348 （1.902-9.941）, 3.442 （1.044-11.341） respectively. Conclusion： The risk factors of PICC related thrombosis among cancer patients included diagnosed cancer for more than seven month, metastases, history of radiation, fat emulsion/ mannitol infusion.
