摘要
目的探讨Th1/Th2细胞及其细胞因子在葡萄膜炎患者外周血的表达及意义。方法采用免疫荧光标记和流式细胞术检测48例活动期葡萄膜炎患者和30例健康体检者的外周静脉血Th1和Th2细胞频数,采用流式细胞微球阵列技术检测血清中Th1型细胞因子及Th2型细胞因子的水平。结果葡萄膜炎患者外周血的Th1细胞百分比、Th1/Th2比值、IFN-γ和TNF-α表达水平均高于正常对照组,Th2细胞百分比低于正常对照组,差异有统计学意义(P<0.05)。IL-4和IL-10表达水平与正常对照组差异无统计学意义(P>0.05)。IFN-γ表达水平与葡萄膜炎患者病情严重程度呈正相关(r=0.48,P<0.01);TNF-α表达水平与葡萄膜炎患者病情严重程度呈正相关(r=0.54,P<0.01)。结论葡萄膜炎患者外周血Th1细胞及Th1细胞因子明显升高,并且与病情严重程度成正相关,Th1/Th2病理性失衡可能是导致葡萄膜炎的重要原因。
Objective To explore the expression and significance of Th1 / Th2 cells and their cytokines in peripheral blood of patients with uveitis. Methods The frequencies of Th1 and Th2 cells in peripheral blood were detected using immunofluorescence labeling and flow cytometry in 48 patients with active uveitis and 30 healthy subjects undergoing physical examination; the levels of Th1 cytokines and Th2 cytokines in serum were detected using flow cytometric bead array. Results In patients with uveitis,the Th1 cell percentage,Th1 / Th2 ratio,and IFN-γ and TNF-α expression levels in peripheral blood were higher than that in the control group,and the Th2 cell percentage was lower than that in the control group,with differences statistically significant( P 〈 0. 05). There was no statistical significance on the difference from the control group in IL-4 and IL-10 expression level( P 〉 0. 05). IFN-γ presented a close positive correlation with uveitis severity( r = 0. 48,P 〈 0. 01),and TNF-αpresented a close positive correlation with uveitis severity( r = 0. 54,P 〈 0. 01). Conclusion There were significant increases in Th1 cells and Th1 cytokines in peripheral blood of patients with uveitis,which were positively correlated with disease severity; pathologic Th1 / Th2 imbalance might present an important cause of uveitis.
