自体骨髓干细胞移植配合高压氧治疗糖尿病足溃疡44例临床观察

Effects of autologous bone marrow stem cell transplantation coupled with hyperbaric oxygen in the treatment of 44 cases of diabetic foot ulcer

目的：观察自体骨髓干细胞移植配合高压氧治疗糖尿病足溃疡的临床疗效。方法对88例糖尿病足溃疡患者进行自体骨髓干细胞移植，治疗组44例进行高压氧治疗，并与对照组44例比较，观察疗效。结果患者经高压氧治疗后，跛行距离、患肢疼痛、冷感及踝肱指数、经皮氧分压、踝肱指数都得到显著缓解。治疗后表皮生长因子（EGF）[对照组：（151．4±5．11）ng／L，治疗组：（171．2±5．69） ng／L]、血小板衍生生长因子（PDGF）[对照组：（79．36±6．24）ng／L，治疗组：（90．80±10．31）ng／L]、血栓素B2（TXB2）[对照组：（129．34±14．86）ng／L，治疗组：（104．15±15．26）ng／L]、6-酮-前列腺素（6-K-PGF1α）[对照组：（78．93±15．98）ng／L，治疗组：（91．15±16．27）ng／L]都有显著改善，治疗组效果明显优于对照组。结论自体骨髓干细胞移植配合高压氧治疗糖尿病足溃疡有很好的临床疗效。

Objective To observe the therapeutic effects of autologous bone marrow stem cell transplantation coupled with HBO in the treatment of diabetic foot ulcer.Methods Autologous bone marrow stem cell transplantation was performed on 88 patients with diabetic foot ulcer,of whom 44 were treated with hyperbaric oxygen （HBO）,and the treatment results were compared with those of the control group,and then, therapeutic effects were observed closely.Results Following treatment,such clinical data as limping,pain in the affected limb,sensation of cold,skin temperature,TcPO2 and ankle/brachial index （ABI）were all alleviated dramatically.After treatment,such data as epidermal growth factor （EGF）[control group：（151.4 ±5.11）ng/L, HBO group：（171.2 ±5.69）ng/L],platelet derived growth factor （PDGF）[control group：（79.36 ±6.24）ng/L, HBO group：（90.80 ±10.31）ng/L],thromboxane B2 （TXB2）[control group：（129.34 ±14.86）ng/L,HBO group：（104.15 ±15.26）ng/L]and 6-K-PGF1α[control group：（78.93 ±15.98）ng/L,HBO group：（91.15 ±16.27）ng/L] all improved obviously,and the therapeutic effects of the HBO group were significantly superior to those of the control group.Conclusions Autologous bone marrow stem cell transplantation coupled with HBO could produce good clinical effects in the treatment of diabetic food ulcer.
