城乡一体化进程中天津居民就医行为差异分析

Analyzing the residents' health seeking behaviors in the process of urban-rural integration in Tianjin

目的:比较城乡居民就医行为差异,揭示城乡一体化进程中居民就医存在的问题。方法:设置疾病认知、疾病处理方式、首诊机构和就诊满意度4个变量来描述就医行为全过程,利用卡方检验和秩和检验的方法比较城乡居民就医行为。结果:在疾病认知、疾病处理方式和医疗人员服务态度满意度方面无显著差异,而在首诊机构和医疗服务类别、技术水平、等候时间的满意度方面存在显著差异。结论:就医行为差异主要源于城乡之间医疗资源配置不均等,其中医疗服务人员的技术水平是影响基层医疗机构的发展的最主要因素。

To comparatively study the residents＇ health seeking behaviors in the process of urban-rural integration in Tianjin.Methods Four variables of disease cognition,disease treatment,first diagnosing hospital and satisfaction on treatment were used to describe the treatment process,and analyzed with Chi square test and Wilcoxon rank sum test. Results There had no significant differences between urban and rural residents in variables of disease cognition,disease treatment and satisfaction on medical staffs,but had significant differences in choice of first diagnosing hospital,and satisfaction on category of medical services,medical staffs＇ technique and waiting time.Conclusion The difference of health seeking behavior results from the inequality of medical resource allocation between urban and rural areas,in which the main factors to influence the development of primary health care institutions was medical staffs＇ technique.
