单孔电视胸腔镜下肺叶切除术19例分析 被引量:4

摘要 目的总结单孔电视胸腔镜(VATS)下肺叶切除术的临床效果。方法周围型肺癌患者49例,19例(A组)行单孔VATS下肺叶切除术,30例(B组)实施传统三孔VATS下肺叶切除术。比较两组的临床效果。结果两组手术时间、术中出血量、清扫淋巴结数目、术后3d的总引流量、术后住院时间均无统计学差异(P>0.05);术后并发症发生率相仿(26.3%vs.26.7%)(P>0.05),但均无出血二次手术、重症感染、支气管胸膜瘘和术后30d内死亡病例。A组患者术后镇痛药吗啡用量少于B组[(16.3±2.1)mg vs.(25.5±6.8)mg](P<0.01)。结论单孔VATS下肺叶切除术临床效果与传统三孔VATS相当,更符合微创胸外科的理念,切口美观、术后疼痛轻。
