503例肾活检的病理类型构成分析

Path ological analysis of 505 cases of renal biopsy

目的:评估本地区近4年肾活检病理谱和患者的构成,探讨其流行病学特征,以便临床更好地进行早期诊断和早期治疗。方法:回顾性研究自2012年1月至2015年12月503例经皮肾活检的病理资料,分析年龄、性别、肾活检指标、临床诊断等。结果:503例病例中,男女分别占40.36%和59.64%,年龄为(37.8±12.8)岁;病理类型:免疫球蛋白A(Ig A)肾病、膜性肾病和狼疮肾炎中性别比较差异有统计学意义(P<0.05);原发性肾小球肾炎399例中,Ig A肾病179例(44.86%)、膜性增生性肾小球肾炎78例(19.55%)、系膜增生性肾小球肾炎63例(15.79%);以Ig A肾病最为多见,其次为膜性增生性肾小球肾炎及系膜增生性肾小球肾炎;继发性肾小球肾炎中,以狼疮肾炎80例(81.63%)最为多见。结论:当前的研究为皖南地区的肾脏疾病的流行病学提供了资料,并有助于临床的诊治。

Objective： To assess the pathological data and patients composition of renal biopsies for the recent four years in the local region and explore the epidemiologic features for early clinical diagnosis and treatment. Methods： The pathological data of 503 percutaneous kidney biopsies recorded in Yijishan Hospital of Wannan Medical College from January 2012 to December 2015 were collected and analyzed retrospectively,including age,gender,indication of kidney biopsy,histopathological diagnosis,etc. Results： In the 503 patients,the constituent ratio of male/female was 40. 36% and 59. 64%,with the mean age of 37. 75 ± 12. 80. There were significant differences between male and female in the histopathological subtypes of Ig A nephropathy（ Ig AN） 、membranous nephropathy（ MN） and lupus nephritis（ LN）（ P〈0. 05）. Among the 399 cases of primary glomerulonephritis（ PGD）,there were 179 cases（ 44. 86 %） of Ig AN,78 cases（ 19. 55 %） of mesangial proliferation glomerulonephritis（ MPGN）,63 cases（ 15. 79%） of mesangial proliferative glomerulonephritis（ Ms PGN）. Ig AN was the most common type,followed by MPGN and Ms PGN. LN（ 80 cases,occupying 81. 63 %） was the most common type in secondary glomerulonephritis. Conclusions： The present study provides data on the epidemiology of renal diseases in South Anhui and will be helpful for the diagnosis and therapeutic management.
