肝素结合样表皮生长因子和C-erbB-2检测对于卵巢良恶性肿瘤鉴别诊断及程度的临床意义

Study of HB EGF,C-erbB-2 test in differential diagnosis of ovarian benign and malignant tumor and determine the meaning of a complex condition

目的探讨肝素结合样表皮生长因子（HB-EGF）和C-erbB-2检测在卵巢良恶性肿瘤鉴别诊断及判断病情程度的临床价值。方法收集该院2014年8月至2015年7月因卵巢肿瘤行手术治疗的患者80例,其中良性卵巢瘤各20例,上皮性卵巢癌40例,Ⅰ期13例,Ⅱ期14例,Ⅲ-Ⅳ期13例。另选取30例志愿者的正常子宫内膜卵巢组织。采用流式细胞术检测HBEGF、C-erbB-2的表达情况;使用免疫组织化学法检测子宫内膜癌和正常子宫内膜的C-erbB-2表达。结果 HB-EGF和C-erbB-2在卵巢癌组织中阳性细胞率显著高于良性卵巢瘤,差异有统计学意义（P〈0.05）;C-erbB-2在正常子宫内膜低表达,在子宫内膜癌的含量比正常子宫表达增高,差异有统计学意义（P〈0.05）;子宫内膜癌的病理分期不同,C-erbB-2表达也不同,Ⅰ、Ⅱ期明显低于Ⅲ、Ⅳ期,差异有统计学意义（P〈0.05）。结论 C-erbB-2蛋白高表达与子宫内膜癌的发生、发展相关,检测C-erbB-2有助于临床评估子宫内膜癌的生物学行为和预后。

Objective To study HB-EGF,C-erbB-2test in differential diagnosis of ovarian benign and malignant tumor and determine the meaning of a complex condition.Methods Totally 80 cases of patients with ovarian tumors were collected from August2014 to July 2015,including 20 cases benign ovarian tumor,and 40 cases epithelial ovarian cancer,13 cases ofⅠ period,Ⅱ period in14 cases,Ⅲ-Ⅳ period 13 cases.At the same time,normal endometrium ovarian tissue from 30 cases volunteers were collected.HB EGF,C-erbB-2expression in ovarian tissue were detected by using flow cytometry.The immune histochemical method was used to detect C-erbB-2expression from 40 examples carcinoma,30 cases of normal endometrium intrauterine membrane.Results The rate of positive cells of HB-EGF,C-erbB-2was significantly higher than that of benign ovarian tumor（P〈0.05）;C-erbB-2for lower expression in normal endometrium,endometrial carcinoma in content was higher than the normal uterus express,the difference was statistically significant（P〈0.05）;Endometrial cancer of different pathological staging,C-erbB-2expression was different also,including C-erbB-2expression in endometrial carcinoma of stageⅠ,Ⅱsignificantly below Ⅲ,Ⅳ period,the difference was statistically significant（P〈0.05）.Conclusion The high expression of C-erbB-2protein is associated with the occurrence and development of endometrial carcinoma,C-erbB-2testing could help to clinically evaluate the biological behavior and prognosis of endometrial carcinoma.
