脑脉泰胶囊联合依达拉奉治疗外伤性脑梗死的临床研究

Clinical study on Naomaitai Capsules combined with edaravone in treatment of post traumatic cerebral infarction

目的探讨脑脉泰联合依达拉奉治疗外伤性脑梗死的临床疗效。方法选取2015年5月—2016年5月在广元市中心医院接受治疗的外伤性脑梗死88例,按照治疗方法不同分为对照组和治疗组,每组各44例。对照组静脉滴注依达拉奉注射液,30 mg/次加入生理盐水100 m L,2次/d。治疗组在对照组治疗基础上口服脑脉泰胶囊,2粒/次,3次/d。两组患者均连续治疗14 d。观察两组的临床疗效,同时比较两组治疗前后格拉斯哥昏迷(GCS)评分、肿瘤坏死因子α(TNF-α)、白细胞介素1(IL-1)、S100B蛋白、髓鞘碱性蛋白(MBP)和血清神经细丝酸性蛋白(GFAP)的变化情况。结果治疗后,对照组和治疗组的总有效率分别为81.82%、90.91%,两组比较差异有统计学意义(P<0.05)。治疗后,两组睁眼反应评分、语言反应评分、肢体动作评分、总分均较治疗前显著增加,血清TNF-α、IL-1、S100B、MBP和GFAP水平均明显低于同组治疗前,同组治疗前后差异有统计学意义(P<0.05);且治疗组这些观察指标的改善程度明显优于对照组,两组比较差异具有统计学意义(P<0.05)。结论脑脉泰胶囊联合依达拉奉治疗外伤性脑梗死具有较好的临床疗效,具有显著神经功能保护作用,具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Naomaitai Capsules combined with edaravone in treatment of post traumatic cerebral infarction. Methods Patients （88 cases） with post traumatic cerebral infarction in Guangyuan Central Hospital from May 2015 to May 2016 were divided into control and treatment groups according to different treatments, and each group had 44 cases. The patients in the control group were iv administered with Edaravone Injection, 30 mg/time added into 100 mL normal saline, twice daily. The patients in the treatment group were po administered with Naomaitai Capsules on the basis of the control group, 2 grains/time, three times daily. The patients in two groups were treated for 14 d. After treatment, the efficacy was evaluated, and the changes of GCS scores, TNF-α, IL-1, S100B protein, MBP, and GFAP in two groups before and after treatment were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 81.82%and 90.91%, respectively, and there were differences between two groups （P〈0.05）. After treatment, open reaction score, language score, body movement score, and total score in two groups were significantly increased, TNF-α, IL-1, S100B, MBP, and GFAP were significantly decreased, and the difference was statistically significant in the same group （P〈0.05）. And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups （P〈0.05）. Conclusion Naomaitai Capsules combined with edaravone has clinical curative effect in treatment of post traumatic cerebral infarction, has significant neurologic protection effect, which has a certain clinical application value.
