超声引导下经皮微波消融治疗甲状腺良性结节的价值 被引量:13

Clinical value of the percutaneous microwave ablation guided by ultrasound in the treatment of benign thyroid nodules

摘要 目的:探讨在超声引导下经皮微波消融治疗甲状腺良性结节的安全性及临床疗效。方法:选取行超声引导下经皮微波消融治疗的甲状腺良性结节患者39例共58枚结节,观察甲状腺结节的消融情况、微波消融后结节体积缩小及并发症的发生情况。结果:39例共58枚结节均行微波消融,一次性完全消融54枚,完全消融率93.1%,4枚再次消融后达完全消融。随访6个月后所有结节体积均较术前缩小,其中25例患者治愈,8例结节体积缩小率〉50%,4例25%~50%,2例〈25%。术后轻度短暂声音嘶哑2例,患侧牙龈及肩部疼痛者4例,无继发性甲状腺功能减低等严重并发症。结论:超声引导下经皮微波消融治疗甲状腺良性结节安全、有效、微创、美观,在掌握适应证的前提下,可在临床应用及推广。 Objective:To explore the safety and clinical effects of the percutaneous microwave ablation(MWA) guided by ultrasound in the treatment of benign thyroid nodules. Methods: Thirty-nine patients with 58 benign thyroid nodules were treated with the percutaneous MWA guided by ultrasound. The nodule shrink and occurrence of complications after MWA were observed. Results : Fifty- eight benign thyroid nodules in 39 cases were treated with MWA ,54 nodules were completely ablated at one-time, the complete ablation rate of which was 93.1% , and 4 nodules were completely ablated after MWA again. Compared with before treatment, all nodules volume shrinked after six months of following up. Twenty-five cured. The nodule shrinkage rate more than 50% in 8 cases,25% to 50% in 4 cases and less than 25% in 2 cases were found. The postoperative mild transient voice hoarse in 2 cases, lesion side gumand shoulder pain in 4 cases and no secondary hypothyroidism and serious complications were identified. Conclusions: The treatment of benign thyroid nodule with pereutaneous MWA guided by ultrasound is effective, safe,less invasion and beautiful, which is worthy of application and promotion under the premise of mastering the indications.
