单侧PVP与双侧PVP治疗骨质疏松性胸腰椎骨折的近期疗效对比

A comparison of the short-term efficacy in osteoporotic vertebral compressive fractures:unilateral percutaneous vertebroplasty versus bilateral percutaneous vertebroplasty

目的比较单侧与双侧椎弓根入路经皮椎体成形术治疗骨质疏松性椎体压缩性骨折的临床疗效。方法 62例骨质疏松性胸腰椎单节椎体压缩性骨折患者随机分为单侧组32例和双侧组30例。评估2组手术时间、X线暴露时间、骨水泥注射量、视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)评分、伤椎椎体前缘高度%、后凸畸形Cobb角度、术后近期及短期并发症。结果单侧组手术时间、X线暴露时间、骨水泥注射量及骨水泥渗漏发生率显著低于双侧组(P<0.01)。2组患者术后末次随访时VAS、ODI评分较术前均显著改善(P<0.05);2组术后椎体高度及后凸畸形Cobb角度均较术前明显改善(P<0.05)。结论单侧与双侧椎弓根入路经皮椎体成形术治疗骨质疏松性椎体压缩性骨折均能获得良好的临床疗效,但单侧椎弓根入路具有手术时间短、X线暴露时间短、骨水泥渗漏发生率低的优点。

Objective To compare the effects of unilateral and bilateral percutaneous vertebroplasty in the treatment of osteoporotic vertebral compression fracture（ OVCF）. Methods A total of 62 patients with OVCF were randomly divided into unilateral group（ n = 32） and bilateral group（ n= 30）. The surgery time,X-ray exposure time,the bone cement dose,visual analogue scale（ VAS）,Oswestry disability index（ ODI）,vertebral height,kyphosis Cobb＇ s angle and complications were compared between two groups. Results The surgery time,X-ray exposure time,bone cement dose and leakage rate were significantly less in unilateral group than bilateral group（ P〈0. 01）. The VAS and ODI scores in two groups significantly improved at the last follow-up（ P〈0. 05）. The postoperative vertebral height and Cobb angle significantly improved in both groups after operation（ P〈0. 05）.Conclusion Both unilateral and bilateral PVP can achieve good effect in treatment of patients with OVCF,but the unilateral PVP has advantages such as shorter operative time,X-ray exposure time and less leakage rate.
