糖尿病肾病透析日血糖变化特点与低血糖风险控制 被引量:2

摘要 目的探讨糖尿病肾病血液透析日血糖变化特点,了解透析过程中低血糖发生的风险环节,制定控制低血糖发生策略,减少低血糖发生率。方法选取2015年12月1日-2016年3月1日我院血液透析科血液透析治疗的糖尿病肾病患者50例作为实验组,同时选取慢性肾脏病肾功能衰竭患者50例作为对照组,分别于透析日透析前、透析后1 h、2 h、3 h、4 h测量血糖变化,并进行分析。结果实验组透析后1 h、2 h、3 h血糖进行性下降,变化幅度最大的是第1 h,同时在第1 h、2 h、3 h均出现低血糖以2-3 h最明显;对照组透析过程中血糖变化幅度小,低血糖发生率低。结论糖尿病肾病透析过程中应在透析前、透析后1 h、2 h、3 h监测血糖,同时加强低血糖症状观察,及时采取措施控制低血糖发生。 Objective To investigate the characteristics of blood glucose change in hemodialysis patients with diabetic nephropathy,to understand the risk ofhypoglycemia during dialysis,develop strategies for control of low blood sugar,reduce the incidence of hypoglycemia. Methods From 1st December 2015 to 1st March 2016,50 cases of diabetic nephropathy treated by hemodialysis in our hospital were selected as the experimental group,at the same time,50 patients with chronic kidney disease were selected as control group,blood glucose levels were measured at 1 h,2 h,3 h and 4 h,respectively,before and after dialysis,and carries on the analysis. Results In the experimental group,the blood glucose of 1h,2 h and 3 h decreased after dialysis,the biggest change is first hours. At the same time in 1 h,2 h,3 h of hypoglycemia in 2 to 3 h the most obvious. In the control group,the change of blood glucose was small and the incidence of hypoglycemia was low. Conclusion In the course of diabetic nephropathy dialysis,the dialysis process should be,monitoring of blood glucose in 1 h,2 h and 3 h after dialysis,at the same time to strengthen the observation of hypoglycemic symptoms,take timely measures to control the occurrence of hypoglycemia.
