心电向量图在高血压防治中左室肥厚筛查中的应用

目的探究心电向量图在高血压防治中左室肥厚(LVH)筛查中的应用。方法选取我院760例原发性高血压患者作为观察对象,将超声心动图(UCG)所测出的左心室重量指数作为诊断高血压左心室肥厚患者的标准。对760例患者的心电向量图进行分析,并且将心电向量图(VCG)和超声心动图诊断高血压左心室肥厚有关的因素进行单个因素以及多因素Logistic回归分析。结果将超声心电图作为标准,心电向量图检诊断的敏感度是71.66%(387/540),特异度是73.63%(162/220),准确性是72.23%(549/760)。对超声心电图以及心电向量图诊断高血压左室肥厚患者分别采用单因素进行分析,将影响左室肥厚患者道德危险因素选出。结论心电向量图筛查高血压左室肥厚患者,不会受到操作者以及仪器等其他条件的限制,具有较高的敏感度,能够很好的应用于高血压社区防治中左心室肥厚的筛查以及随访。

Objective To explore vectorcardiogram in screening of the left ventricular hypertrophy（LVH）of hypertension prevention and control. Methods Selected our hospital 760 cases of primary hypertension patients as research object,the ultrasonic cardiogram（UCG） to measure the left ventricular mass index as a standard of diagnosis of hypertension patients with left ventricular hypertrophy. To analyze 760 patients of vectorcardiogram,and vectorcardiogram（VCG）and echocardiography in the diagnosis of hypertension patients with left ventricular hypertrophy related factors to a single factor and multiple factors Logistic regression analysis. Results The ultrasonic electrocardiogram as its standard,the sensitivity of vectorcardiogram in detection of hypertensive left ventricular hypertrophy was 71.66%（387/540）,specific degree was 73.63%（162/220）,accuracy was 72.23%（549/760）. Of ultrasonic electrocardiogram and vectorcardiogram diagnosed hypertension patients with left ventricular hypertrophy,respectively,using the singlefactor analysis,the moral risk factors in patients with left ventricular hypertrophy was selected. Conclusion Vectorcardiogram in community for hypertension patients with left ventricular hypertrophy of screening work,not influenced by the operator and other conditions such as instrument to restrict. And has a high sensitivity and can be applied to a very good prevention and treatment of left ventricular hypertrophy of hypertension community screening and follow-up.
